Suggest Treatment For Appendicitis With Abdominal Pain And Diarrhea

Question: Hi!
About two years ago, I was in hospital with inflamed appendix. Because there was alot of emergencies there, it took a long time to get decided to do surgery. I was treated at once with antibiotics in the blood. However because of the delays, and the fear of narcosis (I have low bloodpressure, and experience drop in bloodpressure from time to time, and some other issues which no one has been able to diagnose yet.), they decieded to not operate as the inflamation went down nicely. I was discharged from hospital after four days of antibiotics IV, and alot of bloodworks. It looked fine. However, I am still having periods of pain in that aerea. Have taken bloodworks, and CT but nothing seems out of order. It hurts especially after standing upright for a long time. Not the same kind of pain really, as when I was admitted, but surely the same place. I also get periods where my stomach gets "loose" and as soon as i eat anything, I have to use the bathroom, and then its almost like I am "shitting water", excuse my english. All my pains are lower than the navel. It feels kinda irritated and is like that for a week, and the regulates it self back to almost normal. But the area where appendix lies, gets painfull also when my stomach acts up and gets "loose". Other times I can eat and nothing happens.
doctors say it is not inflamed anymore, but might be scartissue that makes the pain after standing and straining for a while.
I am so scared of the narcosis that it is for me the same as dying because of the drop in bloodpressure I experience while for example the dentists, or getting shots, and or when physically straining myself.
I dont want to live like this anymore, and as u see I dread operation.
Do you have any thoughts on this?

Thanks for your query and an elucidate history.
To recapitulate: Male/ - was admitted in a hospital 2 years ago - for days and kept on IV antibiotics - for inflamed appendix - and discharged as inflammation went down - still have problems of pain in the same area - especially on standing upright for long - periods where the stomach gets loose soon after eating, have pain in the same area - watery stools - regulates back to normal after a week - blood work and CT are normal now - Doctor now say that it is not inflamed appendix - and thinks about scar tissue - scared of necrosis, drop in blood pressure and operation ...

My thoughts:
- Since you have now a typical problem of feeling of going to toilet after food- watery, settles on its own withing a week the causes can be as follows:
Appendicitis in spite of normal blood work and CT scan.
Adhesion that occurred after an acute attack.
IBS, irritable bowel syndrome
Diverticulitis and so on.

There are more chances of this being an IBS as per the history you have provided.
Also go for colonoscopy and stool tests for 3 consecutive days, this will help to get a probable diagnosis and gives a plan for further management.
If all the investigations are normal, you may be taken as a case of IBS and treated accordingly.
Also make a list of the foods and beverages that instigate or enhance your problems and stop them altogether.
Control of stress and/or anxiety also has a role in such a case.

Thank you for that.
I just "feel" like its the appendix, as I almost envision the placement of it and thats exactly where I have some numb - ish pain.
Is there any way of treating this without surgery? And what are your thoughts on surgery given that I have this kind of unstable bloodpressure?

I can understand the feeling you must be getting.
If all the investigations are normal the only option remains is a diagnostic Laparoscopy. You have to discuss about this with your General and Laparoscopic Surgeon. This diagnostic Laparoscopy one can explore the whole abdomen, see for any signs of appendicitis under direct, highly clear and enlarged vision and remove the appendix if these are found. This will also give a chance for diagnosis of adhesion and such things where the simultaneous treatment is possible.

Otherwise you can have the courses of prescription based medicines like antispasmodic, probiotics, anxiolytic and so on as per the clinical evaluation, your history and lab findings can help you without operation/surgery.

Unstable blood pressure: you will be evaluated by the Cardiologist and investigated accordingly for diagnosis, treatment and making you fit for surgery if need be. If you have not yet taken an opinion of a Cardiologist, please get one at the earliest.

Okay, thats good to know. Now- I have been checked out by cardiologist.. They took ultrasound of the heart, ECG. They found nothing but some extracystoles and a partial block which he said was not dangerous. He mentioned my bloodpressure was low.
That is what scares me. That I will get heartfailure or arrest after operation as I think anastetics is bad for me. Are the chances for that big for me even if the heart is structually fine?
I really am worried for the operation. Hopefully my appendix settles down or maybe it isnt inflamed after all.

Since your Cardiologist has assured you, stop worrying as the body gets adjusted to the low or high blood pressure (BP) over time.
Secondly the modern anesthesia medicines are very cardio safe and the Anesthetists are well versed and will make it safe for you.

What are the blood pressure and pulse rate readings readings?
Are there any symptoms related to heart or lungs or in general that can be connected to low BP?

Not sure what my bloodpressure was at the doctors office, but I often have readings that are 100/70. Sometimes lower.
Yes, I feel that there are many things related to heart. Such as extrasystoles when I get scared or get adrenalin, dizzyness when standing for longer periods of time, shortness of breath when walking stairs etc. I guess the cardiologist would see if there was something wrong....But he said it all looks fine
I have had a "problem" like this with what I allways think is the heart, for 20 years.
Oh you think a surgery with anastetics wont kill me? :D
It is my biggest fear. I have been avoiding anything that can give me those bloodpressure drops for many-many years! Imagine....I even took risk of not surgery because of it...So now I am just worried that I HAVE to do surgery, and that makes me very depressed, because I feel that is it for me.

Noted your findings and fear, the phobia factor, the anxiety and the stress you are unnecessarily taking.
Surgery with anesthetic will not cause any harm as proper precaustion will be taken by the team of the Doctors if such surgery is needed.
You height, weight and BMI thus calculated also have bearing on the blood pressure.
The phobia factor works much more pronounced than the real blood pressure can give the adverse actual factors as your Cardiologist must have noted.
All these stress and/or anxiety factors can lead to the intestinal problems called IBS and the heart conditions added on to the existing ones.
Get a proper treatment for your anxiety/stress also.
