Brief Answer:
It seems that you will need higher doses of Furosemide.

Detailed Answer:
Welcome to HCM. In light of your medical history, the symptoms seem to be heart failure with buildup of fluid in your lungs and abdomen. You describe what is called PND (Paroxysmal Nocturnal Dyspnea) and orthopnea. Have you noticed that your ankles are getting puffy? Do you have edema or swelling of your shins and legs? The best treatment is lasix/furosemide, and this should be scheduled, then you will need to visit with your cardiologist for optimizing your current medications. I hope that this helps you out, good luck and thank you for your question and feel free to follow up with a question I will be more than glad to elaborate any other questions you may have.
