Question: Hello doctor.. i am XXXXXXX .. My age is 24, and I am facing hair fall problem for last two to three year, and dandruff problem for 6-7 years, earlier hairfall wasnt significant but now I can clearly see my hair density has lowered at top and bald patches at forehead, and also my hairline receded and formed a W shape at my forehead, I also noticed reduced hair at back top of my head, earlier I used to shampoo my hair once a week and rinse daily with warm water and apply coconut hair oil daily and leave it for whole day after taking bath. Now a days I shmpoo daily with herbal anti dandruff khafi shampoo.. And do not apply any oil.. But still at the end of the day somehow time my skin gets oily too much, and my face also become oily, i think somehow my sebum build up rate has increased. It also get clogged up many times and creat acne over my scalp and face(But not that much) I have also consulted to some doctors homeopathic allopathy before, but discontinued their treatment, one doctor asked me to check ige level.. It came out to be quite large around 800 in comparision to to normal level ie below 150.
I also observed hard white buds at my follice maybe due to sebum blockage as in case of acne... But they are distributied evenly not at eash hair root.. But at some places, my father was also bald so maybe my hairfall problem can be hereditary but because of dandruff and high sebum rate I doubt there might be some skin infection or some nutritional imbalance or anything abnormal with my body causing this. I have trued loth of external things like oils and shampoos, out of which Dfree is bit promising for my dandruff problem.. It reduced it to a quite low level but problem is if I drop its use, dandruff comes again.. So now I want to treat everything internally, find the root cause of my problem and get rud of it permanently, at least for dandruff as its quite annoying scratching scalp and seeing flakes. So please suggest me test I should take to find whats wrong with my body. And what treatment I should take.

Brief Answer:
Seborrheic dermatitis with Androgenic alopecia.

Detailed Answer:
Hello, Sir.

I understand your concern.

I have gone through your query in detail.

You possibly seem to have Seborrheic dermatitis.

Also, changes of Androgenic alopecia are present. This is an inherent loss of hair in the pattern you mentioned. In few cases, the process can reverse and attain a threshold where hair loss no longer exists.

Treatment against dandruff will help you prevent hair loss to a large extent.

I generally prescribe my patients Oral tablets of Fluconazole 200 mg once a day for 5 days followed by One tablet once a week for 5 weeks.Additionally Topical application of Candid TV lotion overnight with morning rinse for 3 days a week will clear dandruff to a large extent. Please check with your doctor if he shares my view and if can prescribe these medications to you.

Once the dandruff is controlled, treatment for Androgenic alopecia can be focussed upon.

Post your further queries if any.
Thank you.

Thanx Dr. XXXXXXX

I will ask any nearby doctor to prescribe your treatment.and as dandruff can reoccur so I also want to ask what daily practice I should follow as in diet what I should eat more and what I should avoid, about oiling and shampooing hair frequency and precautions, what oil and what kind of shampoo I should use? And how to use?
Also now a days my face and scalp gets oily too much, is it because of hyper or hypo thyroid, harmonal change?? How can I reduce that??
Coz that make dandruff worse as I noticed.
And should I start to brush my hair?? And massage scalp?? Will that help... Coz I read somewhere brushing will keep skin clean and massaging will increase blood circulation.. But when I massage my scalp lots of hair fall down by root.. So bit confused about it.

Also as you said dandruff treatment will solve my hairfall problems too I also thought the same but My hair have become thin also... Somewhere they turned too much thin..ie At the sides of forehead.. And somewhere they are comparatively thick.. They dont break the fall from root.. Sometime with bulb kind of thing covering the root side end of hair shaft. What are they?

I also had itching and some skin flaking at the chest center, when I showde this too a doc he said your dandruff has reached there... But now its gone after applying Dfree lotion there too.. But I am still doubtfull It may come again in winter... dandruff get worse in those days...

Brief Answer:
Dandruff needs strong treatment

Detailed Answer:
Hello, Sir.

I understand your concern.

I apologise for the delayed response owing to my hectic work schedule.

Oil changes on the face and the scalp is possibly related to underlying hormonal changes.Of course dandruff can worsen excessive oil production.

This will reduce with time as you age. This can be reduced by modification in diet and lifestyle.

Thyroid disorders can also contribute to hair loss and excess oil production.Hence, it will be wise if can check your Complete thyroid hormone profile.

Use mild shampoos like Dove and Loreal.

Shampoo your hair not more than 3 times a week.

Avoid oil massaging.Oil your hair(gently) twice a week only.

Almond oil will suit you best.

Avoid combing wet hair.

Avoid junk, fried and fatty foods.

Drink 3-4 liters of water/day.

Do not brush your hair, until hair fall subsides.

Use Cold air blow dryer only.

Daily intake of high-quality proteins is must in your case-Like Soy, Egg white, Fish and Mushrooms.

Hair fall in your case is in Telogenic phase-The bulb like appearance ar root is hair follicle- the part beneath the skin, when pulled from the skin, called the bulb. This organ is located in the dermis and maintains stem cells, which not only re-grow the hair after it falls out but also are recruited to regrow skin after a wound.

The chest flakes are directly caused by dandruff.Continue D free lotion.

Dandruff needs regular therapy to keep it completely away.Recurrences are very common. But if therapeutic measures are strong and regular, dandruff can be largely controlled.

Post your further queries if any.
Thank you.
