Suggest Remedy For Severe Back Pain

Question: Hi ,
I have been having back pain for 4 weeks now. Initial 2 weeks, I was under 'myozpaz and myozpaz forte' medication. The pain did not subside and i underwent an MRI scan. I have attached the MRI scanning report as well. In the post scan prescription, doctor has suggested the following -

- Myovedic oil
- Belt on the back
- vovenec plus tablets ( 5 days ) twice a day
- Advised IFT, Ultrasound, Spine extension exercises ( PHYSIOTHERAPY ) for 10 days
- Nervijen p ( once a day for 10 days )

My feeling currently -
The pain is quite prominent during forward and backward bending, rolling to either sides during sleeping at night, both legs (hindside - jhun jhun feeling) during sitting, back and underneath the ankle ( sole portion ) pains during walking. discomfort if sitting beyond 5-10 mins. The pain in the back and legs intensifies during the night, making it hard to sleep :(. I sleep with a pillow currently.


So As a second opinion and also to know the problem further,
- How serious is my problem ?
- How strong is the advised treatment prescribed above ??
- What are the main things i need to follow at this time to ensure fast recovery ?
- What are the critical DO's and DONT's ?
- Is it adivesed to drive bike at all ? What about sleeping and sitting posture ?
- How long would it take to completely recover ?
- How much XXXXXXX weight i can carry on my back at this point of time ?
- Where exactly and how tight should i wear the belt on my back ?? SHould it be placed directly and tightly over the affected area ?Currently i keep it on fully from morning till evening.

Disc prolapse with nerve root compression.

Hello,
Thanks for posting your query.
1. You have a disc prolapse with mild nerve root compression at L4, L5 and S1 levels more on the right side. This can be treated with medicines and rest and physiotherapy exercises after the acute pain subsides. Regular physiotherapy exercises are needed to prevent the recurrence.
2. The treatment prescribed to you is just the basic treatment. All the medicines are safe and mild.
3. Discontinue using pillow while sleeping straight. Do not lift heavy objects, especially after bending down. Eat a healthy wholesome meal which should satisfy your minerals and proteins requirement.
4. The following measures will help with the pain
• Lie on a hard bed.
• Correct your posture while sitting
• Avoid prolonged sitting
• Avoid strenuous activity & lifting heavy weights.
• Apply diclofenac gel on the affected area. Warm compresses will also help.
5. Avoid driving bike and prolonged sitting till the pain subsides. You can lie straight.
6. Depends on how well you rest and how properly you take medicines and all precautions. Usually it takes 4-6 weeks to recover with full rest.
7. No weight bearing at all.
8. Your orthopedician or physiotherapist can demonstrate how to wear a belt and it is not loose fitting. You can remove the belt while lying down.

Hi Doc,
Is the nerve root compression to my right or left ?? Since pain is more on the right.

- By warm compresses , did you mean hot water bag application ??

- How far an extent is the myovedic oil applicaiton going to help ??

- Also is very hot water exposure to the affected area advised during bathing ??

- I feel acute pain in my right ankle towards the back and side. during walking as well sitting. What do i do to relieve the pain in the legs ??

- Is the physiotherapy methods advised to me the right set ??? Does it address the bulging and eventually lead to healing of it as well ?? Or is it just for pain relief ?? Is this physiotherapy advised for back only or for legs as well ??

- Is walking daily for about a km advised in my condition ?? Or only bed rest ??

- Could you please point me some good website links for correct sitting and standing postures, since there are pleanty ones on the web and they can be confusing to me.

- Could you suggest some good physiotherapist in XXXXXXX JP XXXXXXX 7th phase area ? I went to one today, but diid not find it good.

Thanks for writing again.
The nerve compression is more on the right side. Yes. Warm compress means hot water bag application.
Myovedic oil is a topical analgesic and will help in reducing the pain.
No. Using very hot water can cause local burns. It is better to use a bottle.
The same pain killer as advised for the back pain will help in relieving the pain in legs.
The physiotherapy advised is correct. It will help in preventing the further progression of the problem and strengthening your back muscles to prevent further herniation. It can revert the already bulged disc only to some extent. You can do physiotherapy for legs also.
No. Only bed rest is needed till the pain subsides completely.
Maintain a proper posture while working, sitting and standing. Keep your back straight. If you work for long hours on the computer, the following link will tell you the posture to maintain while working:

Back strengthening exercises will also help.
It is best to get a good physiotherapist recommended in your area by your orthopedician or search online. I do not know anyone personally in that area as I am not from XXXXXXX
