Brief Answer:
Consult a good neurosurgery or spine surgery center

Detailed Answer:
Hello dear,
I have gone through your question and understand your concerns.
Treatment for disc prolapse not responding to physiotherapy and medicines is surgical.
The centers with neurosurgical facilities and spine surgery unit are adequate for management of such complains.
The decision to go for surgery depends upon actual status, clinical examination and MRI findings.
I will recommend you to get in consultation with a good neurosurgeon near to patient's residence.
There are many hospitals and centers available in newyork and information available online too. It's better to discuss with your colleagues, relatives regarding the appropriate place.
Hope you found the answer helpful.
Do get back with further queries.
