Brief Answer:
Need some info from you ma'am, list of possibilities

Detailed Answer:
Hello ma'am and welcome.

Thank you for writing to us.

I have gone through your query with diligence and would like you to know that I am here to help. I have a few clarifications which will help me rule out a few possible causes, so it would help if you could be so kind as to respond to each one of them:

+Was there some amount of pain in the region even before the shot, and the pain was aggravated by the shot and got worse?
+Could you mention your weight and height please?
+Is there any swelling, oozing of pus, redness in the region where the depo shot was given?
+Are your blood glucose levels well controlled? What was your last HbA1c?
+Any fever off late?
+So the MRI of the lower spine was clear? No abnormalities found?

With the current information at hand, before I receive a reply from you these are the following possibilities I am suspecting:

+Sciatica: But the presentation of pain and location described by you does not match this condition, need a physical examination
+Infection or abscess from the depo shot given, usually requiring antibiotic coverage and/or incision and drainage
+Spinal abnormality (e.g. disc bulge, disc prolapse, etc; causing the pain through your leg)
+Poorly controlled blood glucose levels resulting in poor healing and thus either only pain and/or infection

I look forward to hearing from you in order to help you further. Please do not hesitate to write back to me for any further clarifications, I am always here to help.

Best wishes.
