Suggest Remedies For Severe Depression, Anxiety And OCD

Question: Hello, I'm a 35 year old male. I've been a very happy and effective person my entire life. Suddenly in December of last year i came down with severe depression and anxiety that was caused by intense ruminating thoughts. I've since been diagnosed with OCD with depression and anxiety as secondary diagnosis. I've been in therapy for the last 9 months, have been on a number of meds and have done extensive amounts of neuro-feedback to try and remedy this. Nothing has worked and I'm wondering if there is anything else i need to look into medically? Should i get a brain scan? I'm very discouraged and my once wonderful life seems very hopless. Please help. Thank you very much.

Thanks for writing to us. I can understand your concern.

As such in patients with OCD, depression and anxiety brain scan comes normal. However when disorder is resistant to all treatment, we can go for Brain scan to rule out any rare organic possibility.

To help you further I need more details about your problem.

- Is your problem started suddenly without any stress or event?

- What are your ruminating thoughts?

- What medications (with dose and duration) and therapy have your tried until now? Up to which extent you got relief?

- Have you tried CBT - Cognitive behavior therapy?

- Have you undergone serum Vitamin B12 level and thyroid with routine physical check-up?

- What are current symptoms?

- Have you suffered from double vision, unconsciousness, bleeding from ear -nose-throat or severe unbearable headache ever?

Thank you for the response, it started suddenly on December 12th 2015. My girl friend told me something about her past that bothered me and since then my thoughts have been dominated by thoughts of her past before me. I literally spirled into an abyss. Its the most ridiculous thing i know, but the thoughts are so powerful they literally debilitate me even though it wasnt that bad. And the thoughts can be about anything i make up. Constant rumination. I have done the neuro-feedback. I was also on 100 mg of welbutrin and 50 mg of lexapro for 6 months. My psych put me on 100mg of luvox instead of the lexapro but only took that for about a week before quitting everything. This whole time ive been doing regular therapy every week and I just competed a week long intensive therapy session...so 9 months worth. I have not done the CBT, I'm trying to work on some thought stopping techniques but its to very little avail. They drew blood for my thyroid but i have not heard back on that yet, will hopefully get that tomorrow. No bleeding, super severe headaches, double vision or unconsciousness. Current symptoms are sudden thoughts, followed by anxiety, not necessarily panic, but anxiety enough to make the bottoms of my feet and my palms sweat constantly. Ive been sweating for 9 months now. There is a constant pulse in my head and I'm exhausted and depressed all the time and have no interest in anything i use to care about. Im a very active and passionate person normally and all that seems to have gone. Thanks again for your response.

Thanks for response.

Luvox is good medicine for OCD but 100 mg is not sufficient approximately 300 mg required.

Treatment of OCD includes medicines and psychotherapy.

SSRI group of drugs (fluoxetine,paroxetine,sertraline, Fluvoxamine) are main stay of the treatment. They are antidepressant and in OCD used in high dose.

Benzodiazepines group of drugs(clonazepam, lorazepam or etizolam) are used initially for short duration for fast symptomatic relief.

Cognitive behavior therapy (CBT) is also very useful. It is a type of psychotherapy. It gives excellent results in OCD when appropriately applied.

I used to treat my patients with Fluoxetine and CBT initially. I also add etizolam or clonazepam for short duration. When these do not work I add clomipramine.

You need fluoxetine with CBT according to my opinion. For anxiety short course and as and when required use of benzodiazepine is very helpful.Please ask your treating doctor about the same. You will surely be benefited. You should get result within 4- 6 weeks of sufficient treatment (fluoxetine optimum dose 80 mg).

Thank you again for your response, so i started on 50 mg of fluvoxamine(luvox) today. My psych wants me to work my way up to 200 mg by the end of the week and stay there for the next two weeks until i see him next. I also have a script for clonazepam .5 mg for short term anxiety which i dont use often cause i dont want it to make me tired and i'm afraid of addiction, but might try it more sporadically. I think my psychs plan is to bump me to 300 mg in 2 weeks if im doing ok on the 200 mg. i would like your thoughts on if you think this is a good mix. Also, my psych said it csn take up to 8-12 weeks for the fluvoxamine to take effect on the OCD. Do you think I should see results sooner? My life and relationship would really like to hope so. I was on Lexapro 50 mg and 100 mg welbutrin for 6 months and saw no results, was that cause i was on such low doses? Thanks so much.

Your psychiatrist is doing very good treatment. Even if I were there in place of him, I would do the same. In ocd basically higher dose should be reached faster if no response with previous medicines. Yes 300 mg luvox soon is good strategy. You will see some benefit in 4 weeks of reaching 300 mg dose. He is right that for optimum response we should wait for 8-10 weeks.

I recommend using clonazepam without hesitation. It is very good drug to get symptomatic relief. It is not as addictive as you think. One can stop it without problem through slow tapering.

Escitalopram and bupropion are generally not used in ocd. So not to worry that you didn't get response.
