Question: My wife has been coughing heavily for four days and particularly at night. Symptoms include sore ribs, slightly raised temperature - 98o, feeling fatigued. Any advice welcome. Thanks

In my opinion, you should first get done chest x ray to rule out lung infection in her case.
At old age, lung infection is common. And her symptoms like excessive coughing, fever, fatigue, chest pain (pleuritic pain) etc are more suggestive of lung infection.
So first get done chest x ray.
She may need antibiotic and other supportive drugs.
