Question: husband with MDS now has change in blood count - increase in WBC, MD says possible development of ALL. very ill with pneumonia - scattered both lungs. HB 7, hct 18 on admission to hospital. got 2 units blood, hb now only 8. if it has converted to ALL- how long does he have and what is expected course of progression. YYYY@YYYY dont have anything in writing to show you. total WBC jumped up to greater than 20,000 today. It was low like rest of blood count. has been in hospital 5 days with pneumonia, severe anemia, very SOB at rest, sats in mid 80s. on levaquin since admission and added vancomycin yesterday

Hello,

if it's ALL with pneumonia then he's in immediate danger for his life. Otherwise ALL may even be cured (in 15-20% of patients). The MDS itself may be lethal when pancytopenia occurs. You've told me that he has severe anemia but you haven't mentioned the other cells (platelets, white blood cells before pneumonia). The prognosis of ALL is better when remission is achieved within the first four weeks. Right now treating ALL would be difficult (if possible at all) due to the infection.
