Suggest Remedies For Painful IBS

Question: Suffer from painful iBS, at wits end, comes and goes, now pretty constant. Have read "Ten Percent Human", "Prebotics not Probotics" and "Eat Dirt". Pretty convinced is have gut leak, been life long drinker, stopped cold 2 days ago. Drinking Bone broth instead of coffer. Taking glutamine, cutting out wheat products and dairy products. Wake every day with mild intestinal pain, drink bone broth, maybe have an egg. Pain starts to increase, its a constant ache. Am rearrainging my dies in accordance with Immune gut suggestions. Am afraid that gut is so damaged maybe unreparable. Have had all the tests, CTscen etc. Any hope I may find relief. Have just ordered the Ubiomtest kit. How will that help. Hope you have some encouragement for me.....Thanks, XXXXXXX

I have gone through your query with diligence and would like you to know that I am here to help, and being an IBS-sufferer myself, I think I can provide you with my opinion both as a doctor as well as by being an IBS diagnosed individual myself.

I would like to start by saying that the alcohol is a trigger factors for IBS, and should have been discontinued as soon the IBS diagnosis was established. But either you were not aware or you were struggling to get rid of the habit. But sadly, having IBS itself means we do not have a healthy gut, and with the alcohol being added to the equation it does quite a lot of damage.

But let us put that aside and focus on your future health here, so let me start by listing all the trigger factors that comes to mind, for your reference sir:

+Caffeine
+Popcorn
+Tea
+Chocolate
+Fried food
+Fatty food
+Carbonated drinks
+Milk and other dairy products (especially if you are lactose intolerant)
+Gluten and all products containing it
+Spicy food
+Avoid stress (this is one of the most common trigger factors and should be avoided as much as possible, you can consult a therapist to help with relaxation techniques that may help)
+Oily food
+Taurine (present in many drinks, so check and avoid those drinks)
+Sorbitol (also present in many drinks, so check and avoid those drinks also)
+Some vegetables (e.g. broccoli, cabbage, beans, cauliflower, etc.)
+Avoid large meals
+Skins of fruits and vegetables

Drink plenty of water, and stick to fresh home made food sir.

I also have another recommendation, but this is a course of medications that I advise to my patients who experience continues symptoms of IBS and need relief at the earliest. The medication contains mebeverine and chlordiazepoxide (E.g. capsule meva-C). Take this capsule twice daily for a duration of about a month, even if your symptoms subside. And then slowly wean off the medication, but that is something I can guide you with later on.

This medication will stop the episodes, and help your gut recover which is required. Please speak to a pharmacist at a medical store and request for this. I look forward to hearing from you.

I have tried chloridiazepoxide-clidinis-2.5, but didn't see any improvement. Is that the same? I still have 6 capsules left Or is this different...If I ask the pharmacist, do I ask for Capsule meva-C? Do I need a prescription? Thanks

It does have 1 of the contents, but capsule MEVA-C has mebeverine as well, and you are probably talking about librax?

Yes, ask the pharmacist for capsule MEVA-C, and if he doesn't have it, ask if you can order it and collect it once it arrives.

As far as availability without a prescription goes, I have practised in 3 countries and none of them have asked for a prescription for this medication, so I am hoping you can get it as well; but then again, if you live in the USA (Expect the unexpected).

OK, not available in the US, but found it under name of "Colofac". Questions...do you suggest one capsule a day in the AM,.does it relieve the pain promptly or does it take time to be effective, can I drive after taking it, any side effects, will it have bad interaction with the L_Glutamine digestive tabs I take or the Prebiotin powder is use with my Bone Broth in the AM, or Ambien taken at night, and will it eventually heal my gut so I can go back to coffee, or other foods on the don't ingest list?? Thanks

Hello once again sir.

You can take 1 tablet of colofac and 1 tablet of librax (chlordiazepoxide and clinidium) once in the morning, and once in the evening.

Yes sir, once in the AM and once in the PM for at least 2 weeks.

It does take about a day or 2 for you to start feeling the change, but you should start feeling better soon.

You can drive after taking it, it will not interfere with your concentration.

No drug interactions between L-glutamine and mebeverine, so you can take them at the same time, without worries; or with prebiotic powder either.

Interactions between ambien and mebeverine but only with use over 1 month, so let us discontinue the mebeverine once 1 month is up, okay? So mark the date and discontinue it once 30 days are up.

Your gut needs some normal time to heal, and being honest some individuals do not experience healing of the damaged gut, but some (rarely do). The aim here is to keep it from getting worse, so lifestyle changes and this medication for a while to prevent symptoms is what I am trying to achieve here sir.

You cannot go back to coffee sir, not ever. Please understand that, we both are trying to stop the symptoms; and each time the symptoms occur some amount of damage occurs. So if you do suppress the symptoms and feel better, and then get back to coffee, you will again damage your gut, making things worse with each time your symptoms appear. So I urge you to forget coffee and caffeine in general, completely.

No, you cannot and should not have any of the other foods on the Don't list ever in your life sir.

OK, done, have ordered 30 pills from XXXXXXX ...Couple more questions, Im still constipated all the time, can I continue my use of Linzess, till these pills arrive. Will these pills ease the constipation? and if not, can I take linzess for that problem while also taking these pills? thanks, XXXXXXX

Yes, Linzess will work fine, and you can continue them for sure.

Are you talking about linzess? Linzess should help improve the bulk in your stool, allowing you to pass stools, and prevent further constipation.

But if it was the MEVA-C you were talking about, it works more towards normalising the entire bowel function, so yes, hopefully it treats constipation when required, and diarrhoea also when it occurs (hopefully it doesn't).

I think you can first try the regimen I have provided to see if it helps with the entire problem, and if it does, then we can avoid linzess; primarily because I don't want to keep you on too many medications. But if it doesn't even after about 7-10 days use, then we can throw the linzess in there too.

Thankyou, I don't use Linzess every day, but when I just can't pass, and then it works in about 2 hours, and there is no real bulk just fluid. My sincere hope is the new pills can do something to relieve the pain. So far my restrictive diet, no alchohol, coffee, wheat etc, has done nothing to ease the pain. Also doesn't look like will get the pills for ten days. request a final comment, then guess we wait till I get the pills and XXXXXXX any improvement..... XXXXXXX

But I also feel that if your symptoms are still persisting, there could be something you are still eating or drinking wrong. Because in IBS, the bowel only reacts to things it cannot tolerate. As soon as we stop providing that stimulus, the bowel slowly calms down. So I would also request you to jot down everything you have been putting in your mouth ever since you wake up until you get into bed for a duration of 2 days time (symptoms should be present during these 2 days), and write back to me with that information.

It would be best if these 2 days were tomorrow and the day after, so that you can carry a small diary or notepad along with you through the weekend and write everything down. Better not to be a retrograde study because that way you can miss a lot of items, and it wouldn't be accurate.

4/28 dinner: 2 cocktail onions, small fried grass fed beef XXXXXXX 10 blueberries, 4/29 bfst:warm bone broth, egg over easy small order hash brown potatoes,lunch tuna sand on non glutten bread with mayo, snack of salmon dip with gf bread, cup of bone broth, dinner 5 strawberries, 4/30 bfst:blueberries bone broth, egg over easy and small hash brown potatoes, lunch small tuna sand on gf bread, din bone broth, 5 strawberries. 4/30: bfst: bone broth, egg with small hash brown potatoes, 5 sips of rootbeer later, lunch: small peanut butter sand with mayo on gf bread, dinner: small salmon dip on gf chips, 5 strawberries some blue berries...Overview: I have NO stress in my life except for this painful existence and total boredom, which gin and tonics relieved. I was feeling a little better, not great...then today 2 hrs after peanut butter and mayo sand stomach really started aching. Thoughts: maybe mayo or PB not good, don't seem have any problem with eggs, which eat regularly. Have had no alcholol in a week, think I am following you diet well. Please advise what cool drink (other than water) I can sip on these hot afternoons. I do drink Kefir.........If I need to send additional $ for your continued advise, OK. If the pills from XXXXXXX don't do what am hoping for, don't know what I will do, have tried everything. Hope they get here soon..Thanking you again

3. Mayo can be a question mark, as some people respond well to it, and some do not do so well

As I read further, I saw that you did react to the peanut butter and mayo, which means I was right in suspecting a possible irritation to the mayo. So, it would be best to avoid it.
