Suggest Medicines For High Cholesterol Level

Question: HI Doc,
My name is XXXXX and im 30 years old. I have very high cholesterol. I have been checking every 6 months and my cholesterol level is high. Below are the latest reading can you please guide if I should start medication or Should I exercise.

CHOLESTEROL-------- TOTAL 220 mg/dl ---- CO,CE,peroxidase
TRIGLYCERIDES, SERUM --------- 301 mg/dl -------GPO trinder
CHOLESTEROL, HDL, DIRECT--------37.5 mg/dl -------- Direct Homogenous
CHOLESTEROL, LDL, DIRECT ------- 145.3 mg/dl----------- Colorimetric
CHOLESTEROL-------- VLDL 60.2 mg/dl ---------------Calculated
CHOL HDL RATIO ----- 5.9 :1 --------- Calculated
LDL HDL ratio---------- 3.9 :1 ---------- Calculated

Dear friend,
Welcome to Health Care Magic

You should start medicine.
Your LDL is approximately double the normal limits and you are unlikely to reach target levels without drug. Simultaneously, you should go for life style changes – diet and exercise
Your triglyceride is not very high and will benefit from the same drug

LDL (& non-HDL) is bad cholesterol – lower, the better. HDL is good cholesterol – higher, the better.
In the absence of any other risk factor 100 is ideal for LDL-C. In the presence of ischemia, 70 should be the aim.

     Keep your weight to normal / even a little less
     Quit smoking, if you are a smoker
     Do regular EXERCISE – say walking or jogging - 30 minutes a day / at least 5 days in a week. Isometric exercise (like bull worker) is bad.
     Healthy FOOD is a must. Avoid saturated fats – coconut oil, palm oil, butter, ghee, full cream milk...Avoid junk foods in particular – French fries have palm oil / Pizza has cheese / XXXXXXX sweets are made in ghee.../ Change to Poly-un-saturated oil – like sun flower oil. / Include Mono-un-saturated oil – Olive oil.

Repeat the test in 6 weeks.

Thanks Doctor, can you please recommend the XXXXXXX please. I'm in XXXXXXX now, will be settling down in Singapore this saturday. The drugs are bit costlier there, So can you please reccomend the durgs.

And is my cholestrol is too high and risky ? i consume alcohol almost everyday and also a minimal smoker.

Drinking and smoking should be stopped – IMMEDIATELY / COMPLETELY / PERMANANTLY
Cholesterol / Alcohol / Tobacco are all risk factors – but TREATABLE
Alcohol raises triglyceride in particular…

Statin is the first choice – atorvastatin is time tested and cheap / and is available over the counter..
Start with 20 mg at night
May step up and/or add ezetimibe – after 2 months, if not controlled
