Suggest Medication For Heartburn While On Metformin And Glipizide

Question: I have recently been dx w/ type 2 diabetes and now taking metformin and glipizide. I have been experiencing significant heartburn and trying to find something for relief. What medication can I take?

Thank you for contacting HCM with your health care concerns

You have diabetes being treated with metformin and glipizide and suffering from heart burn or GERD. I would give you the following measures which none of the medicine things that may help reduce your GERD:

1. Elevate the head of your bed with a XXXXXXX This will help gravity keep acid from going into your esophagus while sleeping. It only needs to be done once and it should help.

2. do not eat late at night and do not over eat. When you eat till you are full there is extra pressure on your valve and this will allow acid and food to get into th esophagus and cause burning.

3. Lose weight if over weight.

4. Do not wear tight clothes or pants as this again puts extra pressure on the stomach.

For medications you can use H2 blockers like tagamet or Zantac. These are taking once or twice daily. Regular antacids will work quickly. Lastly you can use PPIs or Prilosec or Nexium. These medication must be taken in the morning 30 minutes before eating. You can not take it while eating as the acid producing cells get stimulated and the prilosec stops then from working but once started it can't be stopped. So 30 minutes before eating and then eat at 30 minutes this alloows the medicine to completely bind to the acid producing cells and helop reduce GERD.

Thank you for your response, Dr. Schubert. Just to clarify, is the recommended preference for medication first be Tagamet/Zantac over Prilosec/Nexium? And is this because of the way the H2 blockers interact with the diabetic meds vs the PPI's ? or is there any preference, at all ?

Detailed Answer:
The reason for the H2 blockers first is that they are less powerful than the PPis and if you can get good results with the H2 blocker then why use the more powerful and expensive PPIs. It like using a sledge hammer on a nail when you only need a regular claw hammer. There are no drug interactions or concerns with taking either H2 blockers or PPIs when you have DM

I have done some independent research regarding Rx interactions and was concerned about taking any medication with my current script for Metformin and Glipizide.
