Question: I am having knee pain problem for last 25 days, walking is OK, but difficulty in walking stairs or climbing or putting weight on joints. All serum tests, RA, HLA B27, etc have out come negative. ESR is 10, Vitamin D,25 hydroxy IS 70. bESIDE KNEE THERE IS STIFFNESS IN BOTH WRIST Joints which persists for about 15-20 minutes. Dr is suggesting 1 gm of Saa daily, 4 mg medrol for 1 week, follwed by 2 mg for 2 weeks, and floitax 2.5 mg alternate days. I am not sure if problem is due to arthritis, or not. Could it be possibly vitamin d3 deficiency. I am confused, i think saaz 1000mg is too heavy for 1 month, pls suggest

Brief Answer:
No SAAZ 1000 mg per day is dose

Detailed Answer:
Hello,
I have studied your case and I think your doctor is right in prescribing these medicines. These drugs are used for multiple joint pain and morning stiffness. So I would suggest you to continue with these medicines. Saaz is given you in right doses. It is given 1000 mg daily for six to 7 months. If there is any side effect then you can discuss with your doctor but most patient receive this well.
Do not take medrol for more than six weeks.Vitamin D deficiency can cause such joint pain so get your blood test to make sure your vitamin D level is above 30.
I hope this answer will be useful for you. Let me know if there is any other follow up questions.

medrol isscribed at 4 mg daily for 1week followed by 2mg for 2 weeks.
doctor also suggests vitamin d injection, 600000 units. I dont know whether this high unit is proper. Or can i take medicines, 60000units every week. is there any diff in taking injection versus vitamin d3 injection. does medrol and methotrexate have any impact to reduce vitamin d3 levels

Brief Answer:
Oral vitamin D can be taken

Detailed Answer:
Hello,
Medrol should be stopped as soon as possible. There is no need to take injection of vitamin D as oral tablet is also sufficient. If there is some problem with your absorption then only injection is indicated.
Medrol or methotrexate has no direct effect on B 12 absorption. It is better from my experience to take oral tablet.
Thanks

thanks indeed.
please also clarify if it could be some other ailment as all RA tests have come out negative so far (RA factor, anti CCP) and ESR is also low, 8. My vitamin D is also in range. In mornings, i have stiffness in both hands and recently pain has also started in wrist joints. Should I continue these medicines in the event it is not RA. Can Saaz, 1 gm tablets daily, also result in joint pain, by usage of about 2-3 weeks. i AM CONCERNED THAT MY INITIAL DIAGNOSIS MAY NOT BE CORRECT.

Brief Answer:
This can be seronegative RA

Detailed Answer:
Hello,
In some cases seronegative RA is there , and most of the blood work is negative.Saaz not causes joint pain. So i would recommend you to continue these drugs for at least 3 months. Avoid eating prickles, curd and juicy fruits. Use warm shower .
Avoid airconditioner. I hope this answer will be useful for you. let me know if there is any other followup questions. thanks
