Question: Sir Myself 38 yr old Male suffering from anal pain testicles pain along with lower abdomen pain and pain on both sides of abdomen. Some changes in bowel movements are also noticed but constipation or diarrhoea is not at all there. All these pains are not continuous but all of them are intermittent. Same problems I faced 7 months back at that time after two months one piles like structure came out from anus and it disappeared on it's own and the other symptoms also. Therefore I did not consult a doctor at that time. Now for the last two weeks I am suffering. What to do sir. Expecting an expert opinion sir. Regards Nandesh

I have read your query and here is my advise.

* Pain lower abdomen, testicles and both sides of abdomen can be due to
various reasons as calculus in the urinary system, constipation related
intestine colic with compression of certain nerves or else.

* Best way to evaluate is a physical examination with a consultant surgeon
and ultrasound of the abdomen & pelvis with expert radiologist ; which will
make us to reach at particular diagnosis.

* Meanwhile following measures can be opted for primary relief

- drink more liquids
- prefer diet with high fiber, green leafy veg, cereals, pulses, legumes, fresh
yogurt
- avoid spices, non veg till possible
- avoid weight lifting if involved
- do not withheld urine for a long period of time
- bulk forming laxative as ispaghula husk or psyllium in case of hard stool
- avoid smoking if consuming

Thank u so much for your speedy reply. Some more queries are there. If it is happening because of the said reasons then y anal pain occurring sir. Because both anal pain as well as testicles pain is there I am totally confused. How anal pain and rectal pain are related sir? When testicles pain comes it radiates to the legs also through thighes. Is there any possibility of anal or rectal cancer ? Hope u clarify

* The sacral and pelvic nerves are having common nerve roots and collateral
supply ; certain cases present with referred pain principle as a stone or
gravels in the ureter gives radiating pain to that side testis via dermatome
nerve supply that is the reason why there is anal pain with testicle pain as
well.

* Another example I can quote that when person has severe constipation ,
there is compression over the urinary bladder and one can not pass the
urine till the colon is loaded.

* The symptoms of rectal and anal cancer are quite different from yours as
gradual weight loss, low grade fever, malaise, bleeding per rectum on &
off, constipation or diarrhea sometimes and others.

Sir Thank u so much for your reply. Before closing I will ask some more queries.Main thing is that I am staying away from my hometown XXXXXXX and staying in a rural village of XXXXXXX for job. The medical facilities here are not so good. So for doing further investigation I have to go to XXXXXXX and planning to go to XXXXXXX in the month of October only. Therefore i am asking you the queries. Now my bowel habits are changed especially the frequency. Now i am going to toilet 3 to 4 times in a day. Is there chances any other serious diseases sir. I am maintaining a good weight between 75 to 80 for the last 10 years. And these pains are not preventing me to do anything I am going to office everyday playing badminton having good family life etc. Still these pains are disturbing too.So i am in a complete dilemma. Actually my wife is about to pregnant si u don't want to share this fear with her. That's y asked to you sir. Right now I am having pain on buttocks also.

- Avoid stress, anxiety.
- Maintain hydration with more intake of liquids.
- Diet should be balanced with essential vitamins, minerals, carbohydrates,

- Avoidance of excess dairy fat, spices and non veg items is recommended.
- Raise intake of fiber, organic fresh fruits in diet with fresh yogurt to supply
natural pro biotics to gut for better digestion.
- Continue your play activities.
- Avoid smoking or alcohol if consuming.
