Should Ecosprin Be Stopped 5 Days Before Cataract Surgery?

Question: Sir i am female aged around 65 suffering from cad(cardiac disease,suffered cardiac attack twice) and recently diagnosed with small vessel disease of brain.Rightnow i am taking ramipril-5mg,ecospirin-150mg,atorvastatin-20 mg & metoprolol 25 mg.Sir doctor has also advised me for cataract surgery and they have told me to skip ecospirin 5 days prior to surgery.Will it be safe considering my other problems.

I am Dr. Dadapeer K, an Ophthalmologist and I answer health problems related to eye.

I reviewed your history.
It seems from the history that you are posted for cataract surgery. You are on treatment with Ramipril, Ecosprin, Atorvastatin, Metoprolol for treatment of your cardiac problems.

Ecosprin is an antiplatlet drug and it prevents clotting and hence prevents blocking of the blood vessels which is the mechanism of hear attack.
But since it prevents clotting it is known to be associated with increased bleeding during cataract surgery and following surgery.
Hence we advice to stop the drug atleast 5-7 days before the propsed date of surgery.
However it is not absolutely necessary to stop this drug before surgeries for cataract surgery. Cataract surgery is associated with minimal bleeding unlike other surgeries in which the risk of bleeding is more.
Hence you need to stop ecosprin in consultation with your physician or your cardiologist and only if your doctor feels it is ok then you can stop the drug.

I have operated many patients with cardiac problem even without stopping the ecosprin and no problem has occurred and many a times no problem will occur by stopping the ecosprin for 5 days and many cardiologists have advised my patients to go ahead with surgery after stopping the drug.
But stopping the ecosprin should be done on a case to case basis if cardiologist agrees for that.

Hence you need to consult your cardiologist or physician doctor before stopping the drug and if he agrees then you can do so.

The other precautions that needs to be taken while operating on cardiac patients is to avoid the use of adrenaline injection either during surgery or with anaesthetic agent.

Do discuss all these factors with your treating doctor.

This is in continuation to my earlier query.As mentioned before I am suffering from cardiac problem and small vessel disease of brain.Recently I have been advised for cataract surgery.
I have been told that the range of lens starts from 8000 to 97000(per lens).I am not sure which lens to opt for.If I pick a lens under 20000 will it do the job or it will be a compromise.Please help me out which lens will be ideal for me .

Thank you for the follow up question.

The cost of the intraocular lens depends on the type, rigid intraocular lenses are cheaper than foldable ones.
The multi focal lenses are costlier ones.
Usually I prefer the lenses in the mid range .
I can advice you to go for hydrophobic foldable intraocular lenses.
Just enquire with your doctor regarding the name and type of lens for Rs. 20,000 and if it is hydrophobic it will do the job.
