Question: My wife is experiencing the following symptoms: Night sweats, chills, weakness, more than frequent urination (sometimes requiring assistance to travel to the bathroom) and sometimes heavy breathing at night. These symptoms are not constant but occur about 2 to 3 times per weak.

She has no fever. She is pre -diabetic an taking the following medications: Trulicity; Atorvastain; Doxazosin; Glimepiride; Pantoprazole and Amlodipine. Her blood sugars and blood pressure are normal.

Thanks for the query.

As per her symptoms, urinary tract infection(UTI) can be a possible cause. UTI sometimes can present without fever.

You can go for a urine culture sensitivity analysis and urine routine study to look for any sign consistent with infection.

If the urine routine is clear, other causes have to be considered. Endocrine dysfunction and hematological problems, infective causes like glandular fever can also cause similar symptoms.

As of now, let her drink plenty of water to keep herself hydrated.

I'm not certain that I follow. Are you saying that UTI accounts for the night sweats, chills, weakness? She has had all of the kidney studies and all appears XXXXXXX consistent with her age. She is consistently well hydrated.

Other than this issue, she is of very good health. This current issue has been present for approximately three weeks.

Hi,

UTI is a possibility only. While working on a case, we will rule out the most possible and easily investigated conditions first.

Considering her age and comorbid conditions UTI is a possibility. You can not detect UTI by kidney studies, a urine study is required to make a diagnosis.

As I said, many other conditions has to be ruled out, in case of diagnostic confusion including tuberculosis and endocarditis. However, we will go through a stepwise approach to reach a diagnosis.

Hi,

As of now, I suggest you go for a urine culture and urine routine to look for any urinary infection.
A complete blood count along with ESR and a peripheral smear examination is also required.
