Is Rasalect Suggestible For Parkinson's Disease?

Posted on Tue, 20 May 2014
Question: Hi, my wife has been recently diagnosed with PD after a prolonged tendonitits at the left shoulder, which affected the left arm. Afterwords she presented tremors on the left hand and left leg. We visited three neurologists, done MRI and DATScan. She tried Mirapexin 0,26 mg and 0.52 mg for five months withought obvious results. Now she runs withought medicine but the tremor is rather stable. The last neurologist suggested therapy with Rasagiline 1 mg daily. Due to the high costs of the original medicine (Azilect) I ordered Rasalect 1 mg which is expected in a week or so. Currently the only obvious symptoms are tremors and fatigue. Do you agree with the suggested therapy and do you believe that Rasalect is equally effective as Azilect?

Brief Answer:
Rasalect is equally effective.

Detailed Answer:
Hi,

Thank you for posting your query.

I have noted your wife's symptoms, and I agree with her neurologist's approach.

Many people with Parkinson's disease (PD) have mainly tremors and other symptoms come later on (after a few years).

Rasalect is equally effective in PD. In India, we mainly use rasalect, as it is easily available and is more economical. So, you can use this drug with confidence and assurance.

I hope my reply has helped you.

I would be pleased to answer, if you have any follow up queries or if you require any further information.

Dear Dr XXXXXXX
Thank you for the prompt reply. I am a little concerned about Rasalect side effects and especially the limitation of food intake (e.g. matured cheeses and food high in tyrosine). What she should avoid during the first days of cure and which side effects we should expect?
Once again thank you
doctor
Answered by Dr. Sudhir Kumar (24 minutes later)
Brief Answer:
These side effects are very rare.

Detailed Answer:
Thank you for getting back.

I have noted your concern, and would like to reassure you that these side effects are very uncommon and there is no need to worry about them.

Yes, it is better to avoid cheese in a person known to have high BP, or on anti-hypertensive medications.

No side effects of note are seen with rasagiline, if started at the recommended dose of 05-1 mg once daily.

Best wishes,
Dr Sudhir Kumar MD DM (Neurology)

Hi, I would like to ask whether DatScan (apart from dopamine indicator) measures the volume of damaged dopamine cells in the brain. If not, can we connect the low level of dopamine with factors preventing it reach the specific place of brain?

Answered by Dr. Sudhir Kumar (4 minutes later)
Brief Answer:
DAT scan measures the amount of dopamine.

Detailed Answer:
Thank you for getting back.

DatScan measures the amount of dopamine in the brain, however, it does not measure the volume of dopamine producing cells.

Volume can be measured by high Tesla MRI, but this information is clinically not useful.

Treatment is aimed at providing dopamine (levodopa) or reducing the destruction of dopamine (ropinirole or pramipexole). This drugs are useful later in the course of illness.

Best wishes,
