Question: Hello. My wife is 48. She can ovarian cancer at age of 35, which was successfully treated by chemo. No reoccurrences since then... I (potential father) am 53. What is the chance of her getting pregnant naturally and delivery a health baby at 48?

Additional info... She did have her first child when she was 23 y.o. Now, at 48 y.o. we are considering another baby... What are other preferred options (i.e. donor egg, IVF,...)?

I understand your concern.
since your wife is 48years , we have to find out the condition of other ovary by ultrasound scan and blood AMH,FSH and LH.
Is she still menstruating.?
usually the ovum development at this age and natural conception is comparatively less.
But still after ovarian function assessment by scan , AMH,FSH AND LH testing, we will be able to come to a conclusion.
If ovarian function is in adequate, you can plan for a donor egg IVF if her uterine parameters are normal.
Mean while you can get a semen analysis done.

Hello.
1. Yes, she has her normal menstrual cycle once a month.
2. She was on birth control pill for the last 7 years, but now is trying to off of them.
3. Am I understanding correctly that we would not be able to use her own egg (since is above 45 years old), and would have to use donor egg provided doctor would determine she should be able to carry a baby from donor egg IVF?
4. What other options do we have? My sperm, donor egg, "substitute" mother to carry a baby?
5. What are the concerns of me (father) being now 53? I heard there might be possibly increased chance of autism? Is that correct? What are the other dangers?
6. What is the expected chance to detect possibly down syndrome while a woman is carrying a baby? 10%? 90%? What age is this test normally done at? 3 weeks? 6 weeks after conception?
7. What are other risks for a baby from 53 y.o. father and 48 y.o. mother?
8. What are risks for mother's health if after all she would be carrying a baby at age 48-49?
9. What could prevent 53 y.o. father from being able to have a baby?

Since she was menstruating till now her ovarian status can be found out by doing day 2 ultrasound scan and hormone analysis of FSF,LH and AMH.If adequate her ovum can be used.
If not ,donor egg with your sperm if morphology is good can be used and IVF ICSI can be done and embryo can be transfered to your wife if everything is normal or in a surrogate.
If you are using donor egg less than 30 years chances of down syndrome is very less.But if using your wifes egg chance of down syndrome increases more than 30 to 40 %.
The marker tests ,with nuchal translucency are done after 10 to 12 weeks.
Anomalies chances will increase when we use your sperm and her egg.But that can be understood after analysing your sperm and embryo by the embryologist.
If your wife doesn't have diabetes ,hypertension and other major medical illness she can continue the pregnancy with regular follow up with out much complications. I suggest you to consult an infertility specialist and discuss after evaluation.
