Question: Good day ,

I am 38 yrs male . My blood creatinine level is 1.34 mg/dl, normal is 0.67 - 1.17 . My urine reports are normal my blood urea level is 20 mg/dl . My doctor recommend me Doppler ultrasound of kidney , which came out to be normal .

I have two 5 mm stones in mid calyx of left kidney . I was operated for 11mm stone in same kidney in 2012. My BP and blood sugar level are normal and have always been normal. I Have moderate build 5' 11'', weight 104 kg.

How worried should I be, my doc say that I am still under observation .He says that definitely something wrong with my kidneys but only time can tell . Anxiety is killing me .

Brief answer.....explained below
Detailed answer....
Hello,
Thanks for the query.
I have read your query and understand your concern.
Although one definitely gets worried by seeing a high creatinine level,I would want to assure you that it is too early to label you as having some kidney malfunction.
Why I say so.....
Your all other tests are normal.
Your blood pressure is normal and you don't have diabetes.
There can be various reasons for high serum creatinine levels other than kidney disorders like.....More muscles mean slightly more creatinine. Dehydration, eating health supplements, eating more meat, consuming certain medicines like RANTIDINE,famotidine, cimetidine,BACTRIM etc can cause slightly higher serum creatinine levels and lastly.....trend of serum creatine levels over a period of time is much more important rather than a single reading.
Other reasons can be some recent illness and obstruction to the kidney flow by kidney stones. You already are a patient of kidney stones.
I do agree with your doctor that you need a constant watch.
Test your creatinine levels every 3-4 weeks for next 5-6 months to know the trend.
Don't stress yourself as I feel you may not be having any kidney ailment.
You should take the advice from a nephrologist who can guide you and manage you properly
I hope it helps, however you are most welcome to revert to me for any further query.
Best of luck
Thanks

Dear sir ,
Good day !!
Many thanks for your prompt reply . Kindly allow me to ask following queries.

1) In your opinion, would it be safe for me to go for surgery to remove stones with elevated creatinine as my job demands absolutely no kidney stones.

2) Assuming my kidney function is normal and my creatinine is elevated due to reaction of medicine/ home remedy. How much time do u anticipate for creatinine to return to normal without any medication and by following strict diet plan .

3) which test you suggest after renal Doppler ultrasound test to ensure that kidney function is normal or Doppler test is good enough to draw this conclusion .

Kind regards
doctor

Hello,
You can definitely go ahead with the surgery for removal of kidney stones.
It is quite possible that you may have a normal creatinine level even in your next test after 3-4 weeks.
Doppler test only reveals the structural damage to the kidney and not exactly the proper functioning of kidneys.
You must also get albumin creatine ratio in the urine.
You may revert for any other query.
Thanks
