Question: I have had constant fevers almost everday since last may, 2019, I have taken my temp to make sure this is true in everyway i thought possible. Im a 19 year old girl. i have taken adderall xr since i was little. i have switched anti depression meds a lot in the past couple of years. my psychiatrist thought my symptoms were serotonin syndrome so i stopped my depression meds. but the symptoms continued. i recently tried a new anti depressent that didnt really help my depression and i wasnt sure if it was making my fever worse so i stopped taking them. ive only been taking my adderall xr since, its been a few months. i get migraines almost daily, mostly at night. i have a lot of other symptoms including getting dizzy when i stand up, feeling hot, sweaty, feelkng like there is something always in the back of my throat, numbness in my pinky and up my arm, racing heartbeat after climbing not many stairs or walking not very far, trouble breathing after walking (feels like im wearing a corset), chest feels heavy, etc. ive got cat scan done which didnt show anything. my doctor wants to do a blood test. i am not sure whats wrong with me. i have tried a lot of thermometers, head ones, ear ones, mouth ones, etc. just to make sure i actually had a fever. i always got fever temps but they were all over the place. i even take my basal temp with an ear thermometer when i feel like i have a fever and it comes out 140 fahrenheit, ive looked this up online and nothing says anything about this and if basal temperatures are usually way high. do you have any idea what is wrong with me, or any tips on how to take an accurate temperature or any suggestions on tests i should get done?

I have gone carefully through your query and understand your concerns. The first thing I would strongly insist on is not to modify the treatment regimen without consulting your psychiatrist. If you think that you need a change in the medication, please do not hesitate to contact him and please do not make changes without consulting him.

The cause of the fevers need to be identified. I would insist on getting a blood count, CRP and procalcitonin levels checked (if not done already) and to share the reports. Also get a chest X Ray done to screen for tuberculosis. I would also insist on sending for a scrub typhus panel. I would insist you to upload that report and also to upload the complete blood count details. If you have the details of temperature spikes, that can really be helpful. I would insist you to share it (if available). Also ask the treating physician whether the liver and spleen are normal or whether they are enlarged. Please share the details. In any case you would need to visit and remain under follow up of a physician so that the ailment can be systematically worked up and resolved.

Based on your inputs I would guide you further.

Thanks for writing back. Unfortunately, that is not how it works. A large number of possibilities can be delineated but that does not mean that you have them. For example, there are hundreds of causes of fever. There are a number of psychiatric issues that you could have. But that does not mean that you have them. A step by step systematic approach needs to be taken to identify the probable cause. You need to visit a doctor for the same.
