Question: wondering how long does it take invega sustenna, anti-pyschotic medicine to get out of system and for side affects to fade? I have not had my period in 6 months. ..Wondering when it will return???

Wondering if the medicine can still making me not have a period after 6 months...I'm still having high prolactin levels. How long will medicine take to be out of system and my period return???

The timing can be predicted by testing serum prolactin levels.

Hi Mam,

I understand your query and concern.

You can monitor the return of menses based on the blood levels of prolactin,which is usually elevated in presence of Invega sustenna.

The normal levels of prolactin has to be reached for normal periods to resume.

The normal values are 2-25 mcg/L.It takes usually 4-8 weeks for the periods to resume once the prolactin levels are normalized.

Thanks for the detailed response...How long does it usually take to get prolactin levels normalized? It's already been 6 months....Can it take up to more than 1 year for prolactin levels to normalized? Also, Dr. told me to take birth control to kick start my period for 1 month...If this doesn't work should I just wait till prolactin levels to normalize which could be another 6 months or more? Very concerned, XXXXXXX K Stanton

Hi Mam,

I understand your concern.

The Drug Invega Sustena leaves the body completely in 3-7 months.
Since you have off the drug from almost 6 months,I will take one more month for the drug to leave the body at Maximum.

The prolactin levels can reach the normal range only after the drug completely leaves the body.

Hence I say that you may start your period in 1-2 months initially as a slight pattern and subsequently as normal pattern later.

OC pills will surely aid you in this process.

But the most important thing is the normalization of serum prolactin levels for the return of menstruation.

Hence I would like to conclude that you may resume your period in 1-2 months from here on.Oc pills will certainly help you in this regard.

how do you know for sure period will return in 1-2 months? The company Jansen that manufactures invega sustenna and risperdol consta said invega can be in body for up to 343 days which would be 11 months not 7....How do you get 7 months?

Thanks so much for this informative email. I look forward for my period to resturn in 1-2months but what if it doesn't? Does that mean meds still in my system? I go back to Dr. end of next month to check prolactin levels that hopefully have gone down. to almost 2-25 mcg/L. Have you dealt with this issue in women taking this invega sustenna before? And did it take 7 months or less for period to come back????

Drug elimintion is based on half life.

Detailed Answer:
Hi Mam,

I understand your query and concern.

The extent of the duration of a drug stay in the body is guided by its half life.It takes almost 4.35times the half life for complete elimination of a drug from the body.

The half life of Invega Sustenna is 49 days,which means it takes 49*4.35 days for elimination which equals to 213.15 days which approximates 7 months.

For Risperdal Consta,which is a long acting preparation,the half life ranges from 3hrs in rapid metabolizers to 20hrs in slow metabolizers.

Hence the range of stay in your body is 3*4.35-20*4.35=13.5hrs-87hrs which accounts for almost 0.5days to 4 days.

Also the incidence of risperdal consta causing hyperprolactenemia is only under 4%.

Since it has been almost 6 months(180 days) ,that you have stopped these medications,once can expect the drug wash out of Invega in another 24 days.After this the prolactin levels take another 2-3 weeks to return to their normal basal levels.

Hence on average it may take 24+21 days=45 days for return of menses.
Hence I suggested 1-2 months for the return of periods from here on.
This is the usual scientific way for calculation of drug elimination,.

I have no clue why your company Jansen mentioned 343 days for both Invega Sustenna and Risperidal Consta.

Yes I have dealt these cases earlier and many of my patients have returned to their periods in 7 months or less.

Jansen mentioned 343 days for Invega Sustenna to be out of system but like you said, it has a half life of 49 days. I am glad you have dealt with other people in taking these drugs...Do you think the birth control will make it take longer to return to menses since it takes 3-6months to return to menses after birth control? Should I stop taking birth control and wait for invega prolactin level to return on it's own. I don't want bc another drug in my system if it's going to take longer for menses to return..What do you think about this?

Were these cases you dealth with before on the exact medicines invega sustenna and risperdol consta???Did any return to periods longer than 1 year or 7 months?

Thank you so much for your responses! I am very worried about these affects these medicines caused me. I hope my period will return soon! I will update you when I get menses back.

So you know for sure my period will return within the next 2months. Everything else was checked out and no other problems were found in MRI or ultrasound. Only thing wrong was prolacin levels were high. So were sure menses will return within next 2 months after prolacin levels return to normal. That would be more than 7 months. Hopefully, your right!!!I don't think I'm ever going to have period return..It's frustrating. But you've dealht with women and these issues before on these drugs and all women had return of period in 7 months or less. So prolactin levels decreased in that amount of time.

Yes certainly and hopefully your periods should return by 2 months.

Taking OC pills can be very helpful at this stage to bring back the periods even much earlier.You need not with held them now.

Yes I have dealt with these cases earlier with drugs equivalent to Invega Sustenna or Risperdal consta.Many women returned to fertility within 7 months or so.

So I advise you not be tensed at the moment.Continue the prescribed OC pills.

If Possible get blood levels checked for Prolactin,meanwhile.

Dear Doctor,

So you are certain I will be getting my period back but it could take at least 2 months or more for it to do so. It is going on 7 months and company said drugs can be in system up to 11 months. I will get blood levels check end of next month as I go back to doctor then. I guess I will continue taking birth control pills till end of month for 28 days as doctor said but not after that.

I am really scared. This is the oddest thing that's ever happened to me before.
I know I'm only 38 and not at menopause as yet so hopefully i will get period back in 2 months like you wrote.

I will be taking birth control pills till the end of 28 days as you directed and my doctor directed. If this doesn't work after 28 days I will discontinue the drugs.
At this point, you and other doctors all agree I should get my period back within next 1-3 months. I go to another specialist on Friday OBGYN to discuss this issue in more detail in person.
