Question: Is it dangerous to leave an inflammed papilliae on the tongue for so long?

they can be cancer

Initially try treatment for few days if not relieved go for biopsy.

Detailed Answer:
Hi Dear,
Have gone through the picture attached by you some possibilities can be-
1)Excessive smoking can cause irritation to the tongue. This can result in enlargement of the papillae on the tongue. So in case you are a smoker it can be due to smoking.

2)Canker sores can be present in the mouth with no known cause.A tissue sample should be taken and should be studied under a microscope for ruling out malignancy.As inflammation is there from a long time possibilities of malignancy should be ruled out initially.

3)Infection may result from trauma of the tongue caused by biting, eating something hot or foods that are too acidic.A broad spectrum antibiotic should be given with anti inflammatory for few days to see if this responds to it.

Some general precautions to be followed are-
-If you are a smoker, stop smoking.
-Avoid eating too much of spicy food and hot food.
-Follow good oral hygiene.
-To alleviate pain caused due to sores, gargle with salt water two times in a day.
-Drink butter milk and eat yogurt.

I do wear a mouth guard at night. Can that have something to do with it?

Hi again.
Yes its very likely that mouth guard is causing constant irritation to that point.
Take a anti inflammatory like Tylenol for 2-3 days and avoid using the gaurd.
See if you respond to it,
