Question: Hi Doctor

I am not sure if you remember our conversation was about 6 weeks ago, I had an exposure with a woman involving deep kissing, i did have a cut on my mouth but no blood, I spoke to you regarding a possible Hepatitis B exposure and testing at 6.5 weeks being negative, since than,I did a Test for all STDS (chlamydia, Gonorrhea, syphilis), they came back negative, I also did a test for the following:

Hepatitis B Antigen- 4 weeks, 6 weeks, 9.5 weeks and 12 weeks - NOT DETECTED
Hepatitis C Antibodies- 4 weeks, 6 weeks, 9.5 weeks and 12 weeks - NOT DETECTED
HIV Test - 12 weeks - NEGATIVE

I have a question, is 84 days conclusive for Hepatitis B, some sources say 6 months, , However my doctor and the CDC website stated 3 months is more than enough time for the antigen to have shown up by now

I did go to the doctors yesterday and received a vaccination for HEP A and B Twinrix - first dose

I am just worried if the antigen hasn't shown up yet will the vaccine work?

Sorry Doctor, one more thing, I did do a 13 week test also couple of days ago it came back negative for HBSAG, however I did take a multivitamin that contained biotin (330MCG) i am not sure if this would affect the test, I had a look and it states it can affect a hepatitis Test but 330mcg is a low dose and i took it 24 hours prior to testing, is it possible this can affect the result?

Brief Answer:
You would be considered negative for Hepatitis B

Detailed Answer:
Hello, it's good to hear from you again, I'd be glad to try to help with your questions. First, 10 weeks is the maximum it should take for the Hepatitis B surface antigen to show up in your blood. So at 84 days, that would be long enough to consider it conclusive that you don't have hepatitis B. So the vaccine would be expected to be effective. If it was earlier and you hypothetically were infected with hepatitis B, getting the vaccine would not be harmful, but it would not get rid of the hepatitis B virus, and eventually your Hepatitis B surface antigen would turn positive. So you can always check the surface antigen again at 6 months, but I don't think that is necessary.

Also, it would be expected that your liver enzymes would be elevated if you had acute hepatitis, so I would take that as reassuring that you are not infected. And the multivitamin should not affect the results of this test, so I wouldn't worry.

I hope that helps, please let me know if you have any questions about any of that, and I would be glad to discuss it with you further.

Hi Doctor

Thanks for the reply, That does provide me with some relief, I took the vaccine 3 days ago, I just hope the antigen doesn't come up positive but from discussing this with different doctors, with yourself and looking at the CDC website, majority would test "detected" for it by now, yes my liver function tests were normal

ALT: 26
AST: 27
GGT:15
Bilirubin: 11
ALP: 49
GLobulin:29
Albumin: 43

I will keep you updated on any changes

Brief Answer:
Everything looks good

Detailed Answer:
Yes, your labs look good, and I would fully expect that you were not infected and that the vaccine will be effective in protecting you in the future from getting the virus. Please do let me know if any other questions come up.

Hi Doctor

Thanks for the reply, I am just wondering, If i take another blood test
For Hepatitis B Serology including the Anti-HBS 16 days after the 1st dose of vaccine, is it possible it can show a false positive for HBSAG?

I have read reports of false positive after people taking the vaccine for a certain period of time?

It has been 6 days so far, I have the urge to take another test at 16-17 day

Brief Answer:
False positives would be rare

Detailed Answer:
A false positive for Hepatitis B surface antigen would be rare, and would not be expected. If you did have a positive, the next step would be a viral load to confirm whether or not you have been infected.
