Question: Hello! this question is actually for a friend of mine, a 53 year old female. She is currently in peri-menopause and due to the severity of her symptoms, she has tried various hormonal therapies. She is looking to see if you have any suggestions on dosage changes or any other hormones, supplements, etc. that could help alleviate her menopausal symptoms without causing breakthrough hormonal bleeding, which she has been experiencing...

These are the various things she has tried:

Progesterone 200 mg and Estradiol patch 0.0375

Progesterone 200 mg and Estradiol patch 0.05

Progesterone 200 mg and Estradiol tablets 1 mg.

All of the above caused breakthrough bleeding for her. She did have a transvaginal ultrasound, which was normal.

She is now currently taking Progesterone 200 mg, 0.5 estradiol tablets. However, she is still experiencing hot flashes and terrible night sweats.

Do you have any suggestions on any dosage changes or anything else that could provide her relief? thank you so much!!

I understand your concenrs.

Following is my reply:

1) She can try oral tablets of estradiol 2 mg instead of patches as patches are not effective in some.

2) Higher dose of estradiol (2 mg) might help in recovering from these symptoms. Hot flushes and night sweats indicate low estrogen in the body.

3) Progesterone in same concentration will do.

4) She might need a prescription for this.
