Question: Hello doctor, i'm 39 years old and recently got my health check done. I wanted some advice on the following: I've attached the report for your reference..

1. T. Cholesterol/HDL Ratio, Direct LDL Cholesterol levels are above normal, I generally follow a good cardio workout session and play soccer. My weight is currently 74kgs.
2. Creatinine levels: My Creatinine levels are within normal but I feel it is still high at 1.25. How can I reduce it? I normally consume non-veg thrice a week and eggs. I've started to drink 4-5 liters of water everyday. Is this good enough to bring down the Creatinine levels?
3. The MCHC and RDW levels are low: I've been taking Autrin tablets twice a week and i've started to consume more greens.

Your cholesterol and low density lipoproteins are high.Your HDL levels are low which are good fats required for metabolism and prevent atherosclerosis.
As your Physical activity is good and weight is controlled,problem must be in your diet.
Avoid fatty,greasy,oily and junk food.
Have raw vegetables and fruits.
Cut down on sugar and refined carbs.
Have apple cidar vinegar,fenugreek and ginseng.
I prescribe such patients of mine,a drug from statin group called Atorvastatin which lowers cholesterol. It is usually started at bed time in a dose of 10 mg and later escalated to 40 mgmt.It cause myalgia and muscle pains in some patients.
I get the lipid profile repeated after 3 months. It should always be fasting (12 -14 hrs) as meals change the values and all reference values are as per fasting status.

Your creatinine levels are normal and you need not worry much.The levels depend on age,sex,time of last meal,protein content of last meals,amount of lean muscle mass etc.
Keep yourself well hydrated.Cut down on red meat,It increases creatinine.Do you have hypertension and diabetes?We need to keep them in control.Reduce your salt intake to less then 4 grams per day.Avoid urine infection and keep good hygeine.
I usually go for Urinalysis of such patients to see if they are losing proteins in their urine and start a drug from ARBs group in low dose to prevent a renal injury.

Your MCHC is low.It is a ratio of haemoglobin to haematocrit means your red cells don't have much of haemoglobin. As haemoglobin is made of iron,increase iron rich foods specially green leafy vegetables (cooked in iron utensils ),jaggery,organ meat etc.
Have more of vitamin c (citrus fruits) as it is required for iron absorption in gut.
You have already started iron supplementation.

Thanks doctor. Regarding my Creatinine levels I would like to confirm that I am not Hypertensive or Diabetic.
I've also done my Urinalysis and the result shows Negative for all of the following: Protein, Blood, Ketone, Nitrite, Luekocyte Bilirubin and Glucose.

Also my eGFR is showing in the Alert mode at 72.0 mL. Can you please advice on this?

It is good that you are not having any comorbidities like hypertension and diabetes.Your urinalysis report is normal so there is no chance of any impending nephropathy.

As far as your eGFR is concerned,you need not worry much about it as it normally decreases with advancing age.
For your age,a value as low as 60 is acceptable.

You just need to have a few precautions.
Keep yourself well hydrated.
Empty your bladder frequently.
Keep perineal hygiene and avoid urinary tract infection.
If you get any UTI,get it treated promptly.
Reduce your salt intake to less than 4 mg per day.
Don't take creatinine supplementation for building muscle mass.
Take NSAID and analgesics only when genuinely indicated.Avoid "analgesic abuse.
Be physically active.
Avoid nephrotoxic antibiotics.
