Question: Hello. My GFR just came back with a level of 57. I do have hx of cysts on kidneys. Shall I see a Nephrologist. Thank you. Traci

LOW eGFR

Detailed Answer:
Normal eGFR(Estimated glomerular filtration rate) is 60 to 90.

If your eGFR is 57,you need to be concerned.

Interpretation of eGFR is dependent on multiple factors like age,hydration level etc and not all patients with eGFR less than 60 are not having a kidney disease.
It is always read in correlation with serum creatinine levels.

Usually eGFR between 45 to 59 is an indicator of mild reduction in kidney function,between 30 to 45 indicates a moderate reduction and between 15 to 30 indicates a severe reduction in renal function.
Kidney failure and need of dialysis is there when eGFR is less than 15.

As per clinical protocols,you need to go for further work up in the form of 2 tests.

1.Routine urine examination to see if you are losing proteins in urine.

Proteins have large molecules and if they are getting filtered,it means that kidney filtration is not optimum.
If there is proteinuria(protein in the urine) associated with it,a eGFR between 60 and 89 may mean early kidney disease.

2.A blood test for creatinine would also reveal the kidney status.
If the kidney filtration is not optimum,the wastes collect in blood and is reflected by high creatinine.

No matter what your eGFR is,you should get it tested again after 3 months in addition to your creatinine levels.
If it is again low,you need to see a nephrologist who would do the clinical examination to look for other signs of kidney problems.

Let me know,if you are a patient of hypertension,diabetes or any other co morbidities.(because these are the commonest culprits to cause renal damage).
If yes,keep your Blood pressure and Blood sugar under control.

You can also take a few other steps to keep your kidneys healthy....

1.Follow a low-salt diet.
2.Maintain a normal body weight and avoid obesity.
3.Take low fat diet.
4.Do regular moderate exercise.Kidney function improves with exercise.
5.Refrain from smoking.
6.Avoid junk food.
7.A high-protein diet should be avoided as it puts a strain on a compensated kidneys to get rid of proteinaceous waste
8.Avoid overuse of pain killers.
9.Don't neglect a Urinary tract infection and get it treated at the earliest.
And above all
10.keep yourself well hydrated.

A kidney cyst is a benign condition and usually do not have an impact on kidney functions.
Most of the times it only causes pressure symptoms and pain.

Hello- At the present time I do have a UTI and am being treated with Bactrim. My creatine level was 1.01, protein total was 6.6. Protein in urine was negative. My calcium levels are 293 a little high. Globulin ( L) 2.3 I have always has kidney cysts I have multiple in the outer pole of my left kidney. Thank you. Traci

I think,you don't need to worry because neither your creatinine is raised and nor you have proteinuria.

Take full course of antibiotics for UTI.

Renal cysts are thin sacs which are fluid filled and are almost always benign.
Usually, they don't cause any problems.
They are quite common in community and most people can go through life without even knowing that they have them.
They are often diagnosed incidentally on an ultrasound.
They don't cause any kidney dysfunction.

As I told you earlier,hence forth you need to be cautious.
Make the life style changes and have plenty of oral fluids and water to avoid dehydration.
Be physically active.
Make dietary changes and have a diet which is low in sodium,phosphorous and proteins.You can talk to a dietitian to make a diet schedule.

Limit foods that are high in potassium like citrous fruits,banana and coconut water.
Limit milk and dairy products.Avoid calcium supplements.
Have fresh fruits and vegetables.
Avoid salt and salt substitutes.
Avoid processed foods and cut on red meat.
Avoid salty snacks,pickles,junk food and canned food.
Have apple cider vinegar and cranberry juice.

Repeat your renal function tests (urea,uric acid,creatinine,serum sodium,potassium and chlorides) and eGFR every 6 to 12 months so as to track the progress of renal functions.

Stay healthy and stay happy.
