Hello Doctor, My Daughter 11 Yrs Old Has Been Detected

Question: Hello Doctor,
My daughter 11 yrs old has been detected with Ns1 positive for Dengue with IGG & IGM negative. She is having high fever 103 specifically starting evening & night from last 5 days. Today the playlet count stands at 22lacs.
While having fever she is also feeling cold in her feet. Alao today she ia complaining about joint pain we are giving paracetamol 500 mg every four hrs.
Pls let me know how long the fever continues in Dengue

Fever in dengue is typically of saddle back or biphasic pattern and usually lasts for about 7 days. High grade fever associated with joint pain, headache and flushed skin with or without rashes are seen during first 3-5 days followed by normal temperature phase for 1-2 days and then reappearance of fever for 2-3 days.
The pattern of fever may vary but usually it does not persists beyond 10 days.
Cold hand and feet is quite common during fever.
The main complication occurs after 4-5 days when fever starts decreasing(at end of first part of biphasic fever) due to leakage of fluid out of blood vessels and fall in platelet count.
If you are noticing swelling in body, decreased urine output, persistent vomiting, rashes in skin, bleeding from any part of body, any altered sensorium or cold hand/feet even when fever is not there please consult your pediatrician.Paracetamol is safe here. Don't give any other analgesic due to its effect on platelets.
Frequent blood investigations and regular follow up with pediatrician is must if child is not admitted in hospital.
Sometimes there may be associated bacterial infection and it should be ruled out by blood test as in that condition antibiotics will also be needed.

Hello Doctor,
Thanks for your advise. Her fever is coming down from high to 100. We are giving are lot of fluids but we need your advise as she has some problem in throat as she is uncomfortable while swallowing & is complaining about pain which is halting her food & fluid intake.

Hi

Its good to know that fever is coming down.Throat pain and difficulty in swallowing in dengue is due to associated sore throat.
Warm saline gargle and analgesic like paracetamol helps.
Here frequent small quantity of fluid intake should be encouraged. Throat pain usually improves witjin two to three days.
Continue routine care and I hope she will feel better within next two to three days.
