Question: Mr doctor ,I'm a 57yrs old ,I'm a math ,I was teaching back India ,but I'm here 25yrs ,2yrs ago I find out I ve suger n high cholesterol ,also high BP, but after 6mo take Medicean 2yrs ago ,I back India n take Ayurvedic med n yoga ,,n I stop all med ,I was taking tab metformin n aspirin n fil oil ,but I thought sickness go ways n I stop Ayurvedic med too ,but 3week ago I become real sick,n see doctor n start metformin n gly buride,but still sick now I ve head ache ,legs pain,n feel this not my body ,plz suggest me what I do next ,thank you kindly mr doctor

I have read through your query in detail.

It states that you were diagnosed to be having sugar, cholesterol and high blood pressure about two years back and have taken medications for a short while before continuing on Ayurvedic preparations which you subsequently stopped.

The present problems have started 3 weeks ago and you were put on blood sugar lowering medications.

Knowing your present sugar levels and cholesterol numbers is important. You might be having uncontrolled blood sugar which is causing the problems like headache, pain in legs and generalised weakness throughout the body.

Right now you must keep a record of your blood sugar levels regularly. In diet you need to make necessary modifications to have a healthy diabetic diet. Please monitor your daily sugar intake. Take your medications as per instructions of your doctor.

As you have high blood pressure, it would be important to get a heart check up done along with an ECG and echocardiography. You have also mentioned taking aspirin, I would like to know any particular reason why aspirin was started for you?

As I know right now my sugar is 165 today morning on 10-05-13
About 700am ,before tea,
Right now I don't know cholesterol ,but high ,as Dr told me ,
BP is 90/145 today , I went to Hindu temple ,there was a medical camps ,there is some things re they chick in n some test they told me to go to lab,
So I will proved u as I get report,
Right now I taking
Tab-Gly-buride 5 mg one a day
Tab-Metformin 1000 mg ,2 time a day
Tab-aspirin 325mg one time a day
One tab a day for BP ,I forget the name ,boz out right now
But not control
I ve all day head ache n legs pain,feet pain ,but not any damage till now ,
As about eyes good but bluere,
I will go to doctor tommarow,

You are welcome and thanks for writing in with an update.

With a sugar level of 165, its slightly on the higher side if considered as fasting blood sugar.
Cholesterol levels also are high as you say.
BP at 145/ 90 is also on the higher side.

Tab-Gly-buride 5 mg one a day and Tab-Metformin 1000 mg ,2 time a day will help in controlling blood sugar but dietary management is equally important.

Aspirin 325 mg is generally given to protect form heart disease and stroke. You may ask your doctor regarding continuing aspirin and the recommended dosage.

Taking insulin must be started under strict observation of your doctor because the correct amount of insulin is calculated after recording regular blood sugar levels. Taking other medicines with insulin is usually not indicated. However it needs to be discussed with your doctor and your sugar levels need to be closely monitored.

It is important that you get your eyes and kidneys checked every year. Diabetes can cause problems in kidney and eyes in many people and getting annual check up is recommended.

Some of the dietary changes that you may bring about are:
1. Eat a lot of non-starchy vegetables, beans, and fruits such as apples, pears, peaches, and berries. Even tropical fruits like bananas, mangoes, and papayas tend to have a lower glycemic index than typical desserts.
2. Eat grains in the least-processed state possible: “unbroken,” such as whole-kernel bread, brown rice, and whole barley, millet, and wheat berries; or traditionally processed, such as stone-ground bread, steel-cut oats, and natural granola or muesli breakfast cereals.
3. Limit white potatoes and refined grain products such as white breads and white pasta to small side dishes.
4. Limit concentrated sweets—including high-calorie foods with a low glycemic index, such as ice cream— to occasional treats. Reduce fruit juice to no more than one cup a day. Completely eliminate sugar-sweetened drinks.
5. Eat a healthful type of protein at most meals, such as beans, fish, or skinless chicken.
6. Choose foods with healthful fats, such as olive oil, nuts (almonds, walnuts, pecans), and avocados. Limit saturated fats from dairy and other animal products. Completely eliminate partially hydrogenated fats (trans fats), which are in fast food and many packaged foods.
7. Have three meals and one or two snacks each day, and don’t skip breakfast.
8. Eat slowly and stop when full.
9. Decrease salt intake to control high blood pressure.
