Does CT Scan For Examination Of Ground Glass Pertain To Me?

Question: Hi, I'm a 44 year old female, ex-smoker and have diagnosed Sjogren's Syndrome.

I presented to my doctor several months ago with night sweats (yes, change bedclothes type), low grade fever (avg. 100 f), and scant hemoptysis. I had a chest CT which revealed 6 pulmonary nodules (pretty evenly distributed) 4 were solid several mm. Two were sub-solid, 6mm. The radiologist and pulmonologist both said to be rescanned in 6 months according to Fleichner Society's recommendations. I have read since that FS published an addendum specifically to address sub-solid pulmonary nodules. If I'm reading this correctly, sub-solid nodules greater than 5 mm should be rescanned in 3 months due to increased risk of cancer. , (I smoked a pack a day from 88-97 and then again from 2012-2013 before quitting a second time. No coughing. I have continued with the fever, sweats, fatigue, and malaise for about 4 months.

My question is the addendum mentions sub-solid and ground glass as being interchangeable. My ct scan didn't specifically say "ground glass" just "sub-solid". Does this addendum recommendation of ct scan f/u in 3 months pertain to me? Thank you.

Hi

Since you are having fever, sweats, fatigue, and malaise for about 4 months and your CT chest reveal 6 pulmonary nodules i will advise for sub solid nodules greater than 5 mm to be rescanned after 3 months and you should do a follow up CT scan after 3 months.

If the nodules goes on increasing in size then you need to undergo a Pet scan follwed by biopsy tset to rule out malignancy.

Hi, I appreciate your answer, but my question was about the Fleschner Society's addendum of recommended follow up on subsolid lung nodules. I have an HMO and so I need to be able to go to my doctor with this rationale for screening before 6 months or he won't approve it and neither will my insurance. Can you please read the addendum linked to above and let me know if that pertains to sub-solid nodules that aren't specifically listed as "ground glass". Thanks

Hi

Subsolid nodules that are not specifically listed as ground glass and is larger than 5 mm requires follow up CT scan examination after 3 months followed by CT exam yearly for 3 years if the lesion is persistant and unchanged according to Fleschner Society's addendum of recommendation.

In your CT chest you are having 6 pulmonary nodules 4 solid several mm and 2 were sub solid measuring 6 mm.

Since one of the size of lesion is more than 5 mm with 6 mm in your case it will also pertain to sub solid nodules that are not listed as ground glass and you have to do a CT scan after 3 months.This will help you to eliminate unnecessary surgery.
