Dear Sir,
My mother has been diagonised with a very slight variation in sugar levels ( fasting sugar of 120; randomised sugar 180). This has happened in the last 1 month. The doctors have advised no tablets but a walk in the mornings and change in the diet ( like eating very frequently, avoiding certain vegetables and junk food). But before this she felt a very bad gastric problem which has not yet subsided. The problem has still persisted since 2 months. She feels very uneasy in the mornings and always wants any one to be with her. The other parts of the day also are very uncertain. We have shown to many doctors but with no clear advice on this. Could you please help us giving a suggestion on this issue?

Welcome to XXXXXXX Thanks for your query.

The blood sugar value of your mother is under normal range. She cannot be diagnosed as diabetic with this value.
So no need of any medication.

But the blood sugar is in the Pre Diabetic range. So in the future your mother has a chance of developing diabetes.
So it is necessary to prevent it. This is the reason diet and exercise advice by the doctor.

Secondly the abdominal problem may be due to Gastritis.
Proton pump inhibitor may help her.
If it still persist then she may need a Upper GI endoscopy to arrive at a diagnosis.

Thanks For you Reply Sir,
Actually we had got her tested 3 weeks ago for the Lipid Profile and Sugar. Then, the sugar levels were 127 for fasting and 233 for randomised.
This week when it was tested, the sugar level has come down to 120 for fasting and 180 for randomised.
But the gas and acidity problem has remained. She says when ever she eats some thing, she gets a grinding feeling in the stomach.

The earlier test is definitely in the diabetic range. The later test is normal.
So clinically the earlier result may be due to stress related hyperglycemia.
In this situation the results are inconclusive. She may need HbA1c and OGTT for a definitive diagnosis.

Stress also may be associated with her gastric problem. Look for any signs of emotional disturbances like depression, loneliness and family related issues.
If so a psychiatric consultation may help to a large extent.

Dear Sir,
We did a test yesterday. Consisting of FBS, HB1Ac, Lipid Profile.

a) The FBS has reduced to 121 from the previous test ( it was 127 previous).
b) HB1Ac is at 6.6% with the graph showing high in the middle of the 1 to 3 month range (upto 1.36% deviation from normal).
c) Lipid Profile shows that that Trigycerides have increased to 261 (it was 233 previously).
Total cholestrol range has reduced from 5 to 3.6.

Please do suggest what should be done for the Trigylcerides. Also, is this the cause of acidity?

HbA1c more than 5.7 % is diagnostic of Diabetes. So your mother is a diabetic and so to be under treatment for diabetes.

Secondly she is having hypertriglyceridemia. This is a harmful fatty acid is causes cerebro and cardiovascular diseases. This has to be treated with lifestyle modification like exercise for 45 min everyday, Dietary increase of fibers like vegetables and greens. Avoiding fatty foods.

Medications include lipid lowering agents like Atrostatins.

Acidity may be due to Gastroparesis due to high blood sugar.
She may try probiotic which will help her in many ways.
