Dear Doctor
I am hereunder giving my glucose levels for over a period of time.

I am 77 yer old . i had diabetes NDDM since 2012.SWince then i am taking Glycomet 500 mg twice a day.I have hypothyroid also. the level is 1.7 5.7 1.8 on last three quarters from 18/12/19. I take thyronorm 75 mcg a day.
My fasting levels are always crossing the normal limit right from the begining. I take two pulkas in the night. nothing else and take 500 mg glycomet. Even then fasting level is more. Kindly advise your opinion in this regard and advise me a good medicine in addition to the exisiting glycomet.Why my fasting levels ae always more. does this due to hypothyroidsm

I have gone through your query and here is my opinion.

As per your reports,your diabetes is fairly controlled and fasting sugar levels are just marginally high.

Please share your diet details and level of physical activity.
Having small multiple meals(instead of large major meals),taking plenty of raw vegetables and fruits,rich fiber diet,abstinence from sweets,alcohol,junk food and food stuffs with a rich glycaemic index should help you to achieve the ideal sugar levels.
This should be coupled with physical exercise and mantaince of ideal body weight.

As far as the drug treatment is concerned,Metformin is a good choice for you as it has a wide margin of safety (looking to your age) and very rarely does it cause hypoglycaemia.

Hypothyroidism has no relation to the aberrant control of your sugar levels but the the drug thyronorm has.

Levothyroxine has significant drug interaction with Metformin.

I would suggest you to increase the dose of Metformin to 1000 mg once a day (in evening) and start taking it in extended release form (XR).
Another option might be 850 mg in morning and 500 mg in evening.
Continue taking Thyronorm in the morning and empty stomach.

It would be better if the modification in your treatment regime is done under the medical supervision and by your own endocrinologist.

Dear Doctor i take very little food for lunch.dhal.rasam and buttermilk with small amount of rice and vegetables.i am a vegetarian . At 1 pm i take my lunch and at 8 pm i take my dinner.in between though not regular i take one or 2 bisscuits.i dont drink or smoke.i walk for 4 km every day since 40 years.iam slim and i am not sedentary type.
i will always on the move.no lethargy. my weight is 63.5 amd bmi is 21 .unfortunately i am averse to fruits. .right from my young age i did not touch the fruits in any form.May i take 850 mg glycomet (xr) in the morning and 500 mg(xr) in the night after dinner? At times i take icecream and cadnurys of course not much.i take coffee once with sugar free tab and greentea twice.this my regimen.kindly advise about medication and any other important advise.do we germt xr tablets comes in the same name glycomet?

You have a good and disciplined life style and dietary habits which hardly need any modifications.
Just keep an eye on diet cheatings as they usually cause a surge in blood sugar levels.
