Can HIV Be Transmitted Through Masturbation?

Question: I am 37 yr old guy. Last night had masturbation with street side transgender but no penetration (vaginal/oral/anal). I My worry is that I do not know whether she was HIV + or not? I am worried now....Please advise.
My main doubt is that she might have come across other customers too if her hand was having some infected semen the? Though she was wth me 10 mins before she did handjob for me.....Please guide. WHat all tests I can do now in XXXXXXX for ascertain and if there is any medicine to start (like PEP)? This thing happened on 22nd may..Frinay night...and today we are on Saturday.....I heard that within 72 hrs to start PEP.......I know I am too worried but do not wish to take chance.

My mind is fully in to it and am getting sleepless nights and nervous days.....PLEASSSE HELP ME OUT.

I have gone through your query and i have understood it.

I don't think there is even a need for testing.
Hiv cannot survive outside body. It dies quickly on exposure to air.
The only possible routes Hiv can be transmitted are unprotected vaginal, anal or oral sex.
Moreover, handjob or masturbation are absolutely safe in terms of risk of acquiring Hiv from her, even if she was Hiv positive.

Thank you very much Doctor. I require your attention to the following:

1.     Before this incident of May 22 Friday night, I already have Allergic Dermatitis since May 18 … Last evening on wards I am on Levocetrizine, Atarax and Clorimazole ointment.. Does at the time of masturbation, would this create any chance of infection? Does it happen only from blood for even with the moistness of Allergic dermatitis. BUT for sure there was NO BLOOD coming from it.

2.     Last night, I sweated heavily and feeling weak. I am feeling general weakness overall and I am not sleeping since last 2 nights. Waking up with all weird thoughts. I think suffering from fear and guilt

My mind is fully in to it and am getting sleepless nights and nervous days.....Is there any probability of this infection? When can I get the test done? PLEASSSE HELP ME OUT.
I have got some red 3 mm patch with blister on my left forearm and left thigh. May be because of Allegic Dermatits but this is haunting my mind. I am equally feeling general weakness/tirednesss too.

Hi.

Your symptoms like weakness and tiredness are due to lack of sleep since you have been anxious and unable to sleep.
There is no risk of Hiv from this particular contact. As I already mentioned handjob and masturbation are not a risk for Hiv.
Whether you had allergic dermatitis or not does not matter here because there is no source of infection in this case. Hiv can only pass through blood, semen, vaginal secretions.
Even if she was Hiv positive she cannot pass it to you with her hands through intact skin of her hands and even if she had infected semen on her hand the risk is negligible because the virus cannot survive outside the body and dies quickly on drying.
Therefore please stop worrying about this and take a good sleep.

But you know I am filled with fear from infection and guilt towards my wife. CAn I have a normal wife-husband relationship with her and normal life with my child?

You are right that since I am not having good and sound sleep hence all these problems and mental worries because of too much google-ing.

I am confident that there was no penetration (vaginal/oral/anal) and no breast sucking. Whatever happened was just she masturbated to me with her hand...I am not sure about her hand but certainly there is no cut or fresh bleeding from my penis (might be fluid of dermatitis then I do not know).

So I do not need any test or PEP...right? If any test then when to have it? Please confirm.

One more thing , last night I consulted Dermatoologist and STD doc for Allergic Dermatitis/Intertrigo....and he gave me Teczine 5 for 7 days, Atarax 10 mg for 7 days and Liquid Parafin and Clotrimazole cream. Here also I told the doctor about my incident and he told not to worry and I am safe ....no chance of any infection.

Can I take this medicine? As I read that steroid reduces immunity.......

Please clarify all my above issues.....PLEASSSSSEEEEEE. Hope you understand my anxiety.

Dear Doc,

I am having really tough time to cope with my mental status and fear psychosis and guilt.

However, I have got 2-3 full vertical lines with red dots from my genital area to mid of the the leg and dark red patches on left and right thighs and tip of nose. May be it is skin issue that I am checking with the doctor.

Secondly, I am feeling totally dull, lethargic, low in energy, vomiting tendency, slight chill at times, dry mouth and no taste of food and febrile and burning eyes. Are these the side effects of the drugs I have been given from May 23 i.e., AF 400 – only 1 tab, Teczine 5 mg for 7 days, Atarax 10 for 7 days and clotrimazole ointment.

I hope this is nowhere related to my fear symptoms of May 22 night. I am 200% certain of no penetration or breat sucking….Might be I touched her front part (transgender) but not touch with my penis immediately). She only masturbated me with her hand that too after 10 mins of talking from her hand. And I do not have private part bleeding except Dermatitis moistness if any. THIS IS KILLING ME DOC.

I do not know how to cope with it. Do I need any test or medicine or psychiatrist?

Please help me out and advise something. Pleaseeeeee Doc.

I reassure you that there is absolutely nothing to worry as far as this particular contact is concerned.
There are also no need to test. However, if you desire you can take a test for Hiv by ELISA at 4 weeks and a repeat test at 12 weeks for a conclusive result.

Anyways, if you are feeling too anxious and unable to focus on other things you can take help of a psychiatrist. No problems with that.

Your skin issue is entirely different. I suggest that you upload a few Images so that I am able to guide you better regarding the skin issue on thigh/ leg.

Dear Doc,

Thanks for addressing my earlier queries. I am somehow unable to take my mind out of it. I am totally restless and getting anxiety and feeling terrible of fear and guilt. Hope you understand. I am sure and telling you true that I never had any penetration with that sex transgender (anal/vaginal/oral). It was purely masturbation and I am paying it now with so much mental anxiety and tough time with all weird thoughts and fear.

I am getting slight cough now and sweet pain at times in lower rib. With the regular usage of Levocetrizine -5 mg still and Clotrimazole ointment, the dermatitis and the other issues are almost gone almost 90% except itching on the penile top portion. Doc has also prescribed me ANZIT 25 mg with which night I am sleeping well but after waking up in the morning, I am getting again weird thoughts doc.

PLEASE ADVISE WHAT TO DO AND WHAT REPORT REVEALS.

CAN I HAVE MY NORMAL WIFE-HUSBAND RELATIONSHIP AS SHE IS JOINING ME NEXT WEEK?

Does Stress make one weak and zombie and loss of appetite? OR is it the medicine? Now even my colleagues at office and known people on phone started telling me that I am dull, non-energetic and not interacting with them and enquiring any problem but I can`t say anything. My physical charm itself is lost with so much anxiety. HOPE YOU UNDERSTAND MY VIEWPOINT.

Thank you for your feedback. For me, I never hide anything from Doctor.

I had girlfriend in my past during my school days in mean 15 years back but I NEVER HAD ANY PHYSICAL RELATION with her.

For the first time I had relation ONLY with my wife and no one till date.

Only on May 22, I had only masturbation with the transgender and no penetration (vaginal/anal/oral). But before that I had allergic dermatitis and it was with itching after applying RINGGUARD and Betnovate N on my own.

It spread upto legs in vertical line earlier for which I am sending you the snap. But after applying RITCH ointment it got cured and with Clotrimazole and Levocetrizine it has now gone down and only 5% is left.

I Never had any relation with any other female Sir and I am true to you.

- But what does the report reveal?

- Has it got any problem relation to my main worry of infection? and does in this condition of dermatitis, does any chance of infection?

- Can I have relation with my wife in normal way?

- Please address to all my worries Sir. I am totally worried, anxious and having tough time coping with myself as I can`t share with anyone.

- Does the report normal and do I need any test further?

Hi.

I can understand your concern and anxiety and I will try to address the issues in best possible way.

The test report reveals that the index for HSV type 2 IgG antibodies is more than 1.1 i.e it is 2.40 which means it is positive.
This test has detected circulating antibodies against Hsv type 2.
That means you must have got Hsv type 2 infection. Only then these antibodies would appear in your blood.

IgG positivity indicates that it is an older infection i.e at least more than 6-8 weeks, rather than a recently acquired one.
Therefore your current sexual encounter with transgender is not the one from which you got this. This infection must have been present earlier.

Hsv type 2 commonly causes genital herpes. It is an STD/ sexually acquired infection.
Infection is usually acquired by vaginal, anal, or oral sex with someone who is infected.

Since the only other partner with whom you have ever been sexually active is your wife. I suggest that your wife also gets tested for this infection.

Rest of the test report is normal.

Dermatitis on leg is an entirely different thing. Herpes infection is not related to HIV. Herpes is caused by a different virus i.e herpes simplex virus Or HSV.

You can have sexual relations with your wife but I would suggest that she also undergoes a test for HSV type 2, IgG & IgM levels.

You may repeat the test i.e ELISA for HSV type 1& 2 (IgG and IgM) to be double sure. Get it done from a different lab.

Yes I ASSURE YOU that whatever I disclosed to you is true.

So, it can be concluded that I should not be worrying about my main concern and I AM FREE from it.

I assume that this particular HSV infection does not get contracted with the masturbation with this transgender on May 22nd night.

AND hence I can have my normal relation with my wife and there is nothing to worry about my main concern and I can be free from my main fear now. Do I need to keep away from my 4.5 year old child too?

Please confirm. Do I need to repeat all tests or only few? Do I need to do ELISA test too now?

It is not possible that you would have contracted HSV by this recent act of masturbation.
Moreover, it is an Older infection not a recent one. I have already told you that IgG positivity indicates an older infection.
You may take a repeat test only for HSV ELISA Type 1 & 2, IgG and IgM.
You can have normal sexual relations with your wife. However, Ideally your wife should also get tested with the same test.
There is no risk of transmission of this infection to your child as this is a sexually transmitted infection.

Hi.

Your primary concern was of risk of HIV from your recent contact with a transgender.
There was no risk of any STD whatsoever from that particular contact with transgender.
Regarding your test revealing Hsv type 2 infection. That is an entirely different thing.
Hsv type 2 usually causes genital herpes and rarely it may cause oro-labial herpes Or cold sores on lips.
The only way one acquires Hsv infection is direct/ close contact i.e sexual contact in case of genital herpes and Kissing in case of Oro-labial herpes. There is no other route for transmission of herpes apart from direct/ close contact.

The reason I asked that I asked your wife to get tested is because she is the only one with whom you have ever had direct/ close contact.
