Unable To Retract Foreskin During Erection Or Clean Penis Head, Skin Irritation. Cure?

sir , i am in a terrible condition , i am not able to pull my foreskin back and it has developed some stiaration / horizontal lines over the skin . also i am not able to clean the head of my penis and the head hurts when touched , when flaccid am able to pull my foreskin manually but when fully errect i, am not able to pull at all
