Orthopaedic Surgeon, Joint Replacement 's  Response
Hi, thanks for writing to HCM.

All the points in your description are suggestive of osteoarthritis (wear & tear) in the knee joint.

Arthroscopy of the knee joint has got both a diagnostic & therapeutic value in your case.

As physical therapy for 12 weeks has failed to provide significant pain relief, I feel that there is no role for conservative management.

Consult an orthopedic surgeon for a detailed examination and definitive management.

Hope this information is helpful. Good day
