Welcome to Health care magic.

1.There is a disc between the vertebral bodies helps as a shock absorber effect, the disc contains maximum percentage of water, due to degenerative changes when the water content / hydration is lost the disc slips / bulges.
2.In your cad the disc involved at the level of L4-5.
3.At L4-5 there is bulge in the disc causing the pressure effect on the nerve which passing through the foramen laterally.
4.With mild compression on the spinal chord. Schmorl's nodes are when the disc bulges in to the vertebral body itself - this condition explained as so.
5.In this condition treatment will be simple back exercise followed by physiotherapy treatment. Good luck.

Hope it helps you. Wish you a good health.
