Welcome to health care magic.

1.The report suggest that there is slight scoliosis - bent of spine to wards left side - which can also be a positional issue or back pain with muscle stiffness might give this findings.
2.There is a slippage of L 5 vertebrae over S1 with needs correction depending on the grade.
3.There is disc bulge - happens due to loss of water content in the disc due to degenerative disease, post traumatic conditions.
4.At the level of L5-S1 the disc bulge is see causing pressure effect on the nerve root.
5.In this case depending on grade procedure might or mightn't needed, for back symptoms - simple back exercises and physiotherapy will be helpful.

Good luck.
