What Do Irregular Periods And Spotting In A 39 Year Old Suggest?

Salam, i am 39 years old and i am un married. I am facing the problem of irregular periods for the last 7 years. Spotting started one week before the periods begins. I never missed a period in any month during this problem. Last year my ultrasound showed one cyst measuring 4.7 mm in my left ovary. By taking medicines , the cyst has vanished. Now dr. wrote in her report that i have multiple follicles in both of my ovaries and it is normal condition and need not to be worried. Kindly suggest me is this normal or risky. Is it a sign of infertility? If yes then what should i do? Should i get treatment to cure these follicles? Thanks .

General & Family Physician 's  Response
Hello,

Presence of cysts in the ovaries can be a normal finding but in some cases may indicate PCOS (polycystic ovarian syndrome). This is characterized by fewer ovulations and may decrease the chances for pregnancy. On the other hand, there is treatment to induce ovulation in case you would like to get pregnant, so there is no need for treatment at the moment.

On the other hand, you may have combined oral contraception to help your periods become more regular and if they do not improve this, you may need to have a transvaginal ultrasound, pap smear and possibly in rare cases even endometrial curettage and biopsy if a cause is not identified.

I hope this answers your query. Let me know if I can assist you further.
