What Could Cause Poor Appetite, Cloudy, Dark Urine With Odor, Purulent Drainage From Urinary Meatus In 88 Yrs Old?

An 88 year old has a poor appetite and consumes less than 1000 mL per day. He has an indwelling urinary catheter due to urinary retention. there is 150 mL of urine in the drainage bag. The urine is cloudy, dark gold and has a strong odor. There is also a small amount of purulent drainage from the urinary meatus. The client complains of lower back pain. His vitals are as follows: temperature is 100.8 F, pulse is 98 and regular, resp. rate is 18 and regular, and blood pressure is 130/90. What conclusions can you draw about the client s physical and psychological condition based on the data provided?

Hi. Thanks for providing a very good history and findings.
Due to the reduced intake the urine gets concentrated giving cloudy, dark gold urine with a strong odor.
The indwelling catheter is a foreign body and a source of infection, giving fever and purulent discharge around meatus and may be in the urethra and whole urinary system.
This may be the reason for lower back pain and poor appetite.
This is about the physical condition.
How can one comment upon Psychological condition as you have not provided any data or information. But you can understand better what would be the condition in a 88 year old male .
