What Causes Persistent Headaches With Ear Pain?

My daughter is 4 years old and has been complaining of a headache for 3 days. She says it hurts in the top of her head and last night she complained a bit of her ear hurting. Her highest temp was 101.5. I have given her motrin and tylenol but the headache still comes back.
