What Causes Persistent Fever?

My 23 year old son has the flu now for 7 days with high temperatures of over 38 degrees for the last four days. He has been prescribed antibiotics of TW-Amoxycilln on Monday afternoon, it is now Thursday nite and he still is getting high temperatures. Should I be concerned? He is an appetite still.
