What Causes Difficulty In Swallowing With Hoarse Voice?

I have had two falls in the past two years and now there is a problem with burping related to swallowing and at times my voice becomes hoarse and I seem to run out of the ability to talk. Also medications cause the soft palate to drop. Could the vagus nerve be traumatized? Barbara
