What Causes Constant Shoulder Pain?

hi, my husband has been having constant, sometimes severe, pain in his shoulder. it started about a month and a half ago. sometimes, he has a sharp, stabbing pain that radiates to his elbow. there doesn't appear to be any sign of "fever" in the joint. when in bed, if lies flat on his back, there is no pain, but if he lays on either side, there's pain. it gets worse as the day progesses. i have given him aleve 220mg, 2 bid for a week with no relief, then i switched him to 800mg of ibuprofen tid, with some relief but we have to be careful with the amt of ibuprofen he gets because he has gerd. he has a gout attack once last year as well.
