What Causes Bruise On The Birthmark?

I have a small birthmark, about 1/4 in diameter, it s on my back around the area of my lower ribs. I have a pretty good sunburn going. As I rubbed aloe on I went over the birthmark and almost fell over. It felt like a terrible bruise, though I can t see one. I m on Xarelto for DVT s. Should I be concerned?
