What Causes Abdominal Distension And Ankle Swelling In An Alcoholic?

my stepfather is a severe alcoholic. I literally have never seen him drink anything but alcohol in 16 years. He now looks like he is pregnant and his ankles are huge from swelling. I know he doesn't feel well, but now is drinking more than ever. Should I be worried anymore than normal?
