What Are The Possible Treatment For Hematoma On Prosthetic Knee? Is Surgical Removal Advisable?

What are the possible treatments for a hematoma on a prosthetic knee in a patient, who was on coumadin when the trauma occurred. He is currently off coumadin and was administered platelets and vitamin K in the hospital, which coagulated the pooled blood and now is not able to be aspirated. Rest and foot elevation is the only treatment currently prescribed. Is surgical removal of the clot possible and when advisable?

Orthopaedic Surgeon, Joint Replacement 's  Response
Hi, thanks for writing to HCM.

A hematoma of small size will eventually gets absorbed within 3 weeks for which an elastic compression bandage is all that is necessary.

But if it is large enough and has occured in a prosthetic knee, then it has to be evacuated by surgical means. This is because of the fact that the hematoma serves as a good breeding medium for the bacteria to proliferate which is not desirable in case of a prosthetic knee joint. This is still more significant if the patient is also a Diabetic.

So, I suggest you to act swiftly and get the hematoma drained (if it is large enough) to avoid the chances of getting infection inside the joint.

Hope this information is helpful. Good day
