Diabetes nephropathy is one of the microvascular complications of diabetes which happens in long standing diabetes or uncontrolled diabetes.
Secondly In the earlier stages of diabetes, there wont be any signs of diabetic nephropathy because clinical signs develop after decline in the glomerrular filtration rate.
The earliest marker of nephropathy is urine microalbumin and creatinine ratio which needs to be checked in every diabetes patient once in 6 months.
The earliest way to prevent the diabetic nephropathy is to start ACE inhibitors at low doses and gradually titrate it to maximum tolerating doses. If ACE inhibitors are not tolerated then ARB group of drugs can be started. Irrespective of the blood pressure recordings.
It will slow down the progression of the protienuria.
