Suggest treatment for anxiety disorder

i suffered from anxiety for 10 years. i was on medication for a few years then went off and was totally normal for 2 years but i feel like its comingback. like im loosing control of myself! im having bad phobias and getting nervous when im away from home. please help me i hate this anxiety what can i do
