Hallow Dear,

I have read all your questions and let me answer them one by one.

1. By just looking at the vaginal opening you can come to know whether the hymen is ruptured or not. Unruptured hymen looks like a membrane with an opening in it just inside the opening of vagina. When it ruptures, its ragged margins are seen. Now a days, it is not always that the hymen ruptures at the first intercourse. Now a days, women are involved in so many activities like cycling, tree climbing, horse riding, athletics, motorcycle riding that their hymen is ruptured during this activities. Sitting astride is sufficient to break the hymen. Hence bleeding at first intercourse is not seen now a days. Intact hymen is no more considered as the sign of virginity now.
2. Even if the semen is deposited at the vaginal opening outside the hymen, the sperms can swim up through the uterus to the discharged egg to fertilize it.
3. The pain you are getting at the penis foreskin area may be due to its inability to get retracted completely. Absolute inability for the foreskin to retract is known as Phymosis. You may be having partial phymosis. Please report to the surgeon. For partial phymosis also, you may have to undergo circumcision surgery; i.e. excision of the foreskin.
4. Two times intercourse a day is a good score and naturally penis will become rather placid. You need not exert too much.
5. The penis after ejaculation always becomes placid. After every intercourse, some amount of semen always flows out. However it is not all the semen that is flowing out. It is about 20% semen flows out. Majority of the semen in the first spurts directly enters the uterine cavity which can impregnate the woman. Do not bother for out coming semen if it is less than one ml.

I hope these answers will remove your misunderstandings and remove your stress.
