Suggest Medication For Depression And Sleep Disorder

hi, I am in need of some medical help regarding depression and a sleep disorder however I am unsure if I can approach my doctor on the NHS without the issue being recorded on my medical records, am I able to ask the doctor to talk off the record about my conditions?
(I am applying for US visas which can be strict when it comes to medical and mental health issues for approvals and I cant afford to risk approval of the visas)
