You did not tell how much time has elapsed since then, but you are able to write your query, then you are lucky that
shock has not done so much harm to your organs, like heart,
kidney and brain, but you must be watchful on your consciousness, orientation, heart beat and urine output.
Please drink more water.
But by the hit on your back, you are in difficult respiration, because you have
chest injury, that might be
rib fracture or injury or any inter-costal (between the ribs) muscles injury.
Intercostal muscles injuries are also very painful and cause difficulty in respiration due to pain and recovered gradually over the weeks.
So you have to rule out any serious chest injury by a simple investigation i.e. chest x-ray, which will be very helpful to find a rib fracture or any hydro or pneumo-thorax, which can cause endanger for your life.
So do not wait to go for a x-ray and meet to your GP for examination and management.
